A 737-200 ON APCH LOST CTL OF #1 ENG. ENG THRUST LEVER WOULD NOT DECREASE PWR. ENG SHUTDOWN. THRUST LEVER TELEFLEX FOUND NOT SECURED.
Narrative
ACFT XXC HAD A DOUBLE ENG CHANGE. THE ENGS WERE REMOVED ON THE XA AND THE NEW ONE WERE HUNG ON THE XB. THE ENGS THAT WERE HUNG WERE ROBBED FROM ACFT XXXF. ONE CREW DROPPED THEM AND WHEN THEY WERE DONE ANOTHER CREW HUNG THEM. I WAS ON THE CREW THAT HUNG THEM ON THE XB; WE HUNG #1 ENG. UPON COMPLETION ANOTHER MECH AND I PERFORMED THE THROTTLE RIGS; ME BEING ON THE ENG SIDE. ONCE FINISHED WITH THE RIG I TIGHTENED THE NUTS AND BOLTS AND SAFETIED THE PART PWR STOP AND ROD ENDS. ANOTHER MECH THEN INFORMED ME THAT WE NEEDED NEW NUTS ON THE BOLTS; BUT SAID THAT HE WOULD DO IT; TO GO HOME. THE FINAL INSPECTION WAS TO BE PERFORMED AND ANTI-SABOTAGE COMPOUND WAS TO BE APPLIED BY AN INSPECTOR. ON LEG 6 OF THE ACFT'S FOLLOWING FLTS; THE ENG WOULD NOT SPOOL DOWN ON DSCNT; SO IT HAD TO BE SHUT DOWN. UPON INSPECTION OF THE ENG AT THE GATE; THE THROTTLE LINKAGE BOLT WAS MISSING AND THE FUEL CUT OFF HAD NO SEALANT ON IT. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE CONDITION OF THE FUEL CTL RIGGING WAS ALL COMPLETED; SAFETIED AND INSPECTED WITH SEALANT APPLIED TO THE BOLTS. THE RPTR SAID 3 INDIVIDUALS WITNESSED THE COMPLETED RIGGING WORK. THE SYS WAS THEN CHKED FOR OP AND CHKED OK FOR SVC. THE RPTR STATED LATER ON; SOMEONE DID SOME WORK IN THE AREA OF THE TELEFLEX CONNECTION BUT DID NOT MAKE A WRITE-UP FOR REINSPECTION AND RESEALING THE SAFETIED NUTS. THE RPTR SAID THE TELEFLEX NUT BACKED OFF CAUSING LOSS OF ENG CTL. THE INCIDENT IS UNDER INVESTIGATION BY THE COMPANY AND HAS REACHED NO FINAL DETERMINATION.
