AN EMB 120 FLC IS MANDATED TO RTN/LAND WHEN THE GEAR FAILS TO RETRACT DUE TO THE GEAR DOWN LOCK PINS NOT BEING REMOVED DURING PREFLT AT SLC; UT.
Narrative
MY CAPT AND I WENT OUT TO THE ACFT TO PREPARE FOR OUR FLT. MAINT WAS AT THE ACFT REPAIRING THE FIRST OFFICER'S SEAT. WHEN IT WAS COMPLETED; WE WENT ABOUT PREPARING AND DID A NORMAL PREFLT INCLUDING CHECKING FOR THE GEAR PINS IN THE GEAR PIN STORAGE BAGS. ALL THREE FLAGS WERE THERE. WE DEPARTED TO POCATELLO AND WHEN THE 'GEAR UP' WAS CALLED FOR; THE GEAR DID NOT RETRACT. WE RTNED TO SALT LAKE WITHOUT INCIDENT. WHEN MAINT CAME; THEY HAD FOUND THAT THE GEAR PINS WERE STILL IN PLACE. THE PLANE HAD COME FROM A FERRY FLT AND THEY HAD SAFETY WIRED THE PINS BUT REMOVED THE FLAGS. THE FERRY PERMIT WAS NOT IN THE AIRPLANE; AND WE HAD NO INDICATION THAT THE GEAR PINS WOULD BE A PROB. I FEEL THAT THIS PROB WAS CAUSED BECAUSE BASED ON PERCEPTION; I DID NOT SEE ANY REASON WHY THERE WOULD BE GEAR PINS PINNED ON THE AIRPLANE. FIRSTLY; WHEN WE WALKED UP TO THE AIRPLANE; THERE WAS A MAINT PERSON FIXING A PROB WITH A SEAT. NO ONCE DID HE MENTION THAT THERE HAD BEEN A PROB WITH THE GEAR. THEN; THERE WAS A WRITE-UP IN THE MAINT LOG THAT SAID; 'LEFT LNDG GEAR 'A' GREEN DOES NOT INDICATE DOWN AND LOCK. TESTS OKAY.' THIS TYPE OF WRITE-UP DOES NOT REQUIRE TO PIN THE GEAR AND FERRY IT. LASTLY; THERE WAS NO FERRY PERMIT ON BOARD. THE TWO SIGNS THAT WOULD INDICATE THAT THE GEAR PINS WOULD BE IN THE GEAR ARE THAT THE ACFT HAD BEEN FERRIED; OR THAT THE ACFT HAD COME FROM MAINT. THE FACT THAT THE GEAR PIN FLAGS WERE IN THEIR NORMAL LOCATION MADE ME BELIEVE THAT THIS WOULD NOT BE A PROB. I DID NOT KNOW THAT THEY WOULD DETACH THE FLAGS FROM THE PINS. I BELIEVE THIS COULD BE AVOIDED IN THE FUTURE IF; ONE; THE FLAGS ARE PLACED IN A SEPARATE LOCATION WHEN THEY GET DETACHED. SECOND; THERE IS A MAINT WRITE UP THAT THE PINS WERE SAFETY WIRED IN AND THIRDLY; THE FERRY PERMIT REMAINS ON BOARD THE ACFT THROUGHOUT THE DAY THAT THE FERRY WAS FLOWN. SUPPLEMENTAL INFO FROM ACN 535203: ON THE PREVIOUS FLT; WITH A DIFFERENT CREW THE ACFT HAD A GEAR INDICATION PROB IN SUN VALLEY; IDA. MAINT ELECTED TO FERRY THE ACFT TO SALT LAKE CITY FOR REPAIRS. I WAS UNABLE TO TALK TO THE CAPT OF THAT FLT; AND HE REMOVED THE FERRY FLT PERMIT FROM THE ACFT.
