A COMMERCIAL MEDIUM TRANSPORT WAS DISPATCHED IN NON COMPLIANCE WITH THE LOGBOOK NOT SIGNED BY A FLT TEST CAPT AND NO TECHNICIANS SIGNOFF.
Narrative
UPON LNDG IN ZZZ; OPEN MAINT DISCREPANCY FROM 2 DAYS PRIOR WAS DISCOVERED CONCERNING A REQUIRED TEST FLT AFTER AN INFLT ENG SHUTDOWN. THE CAPT COMPLETING THE TEST FLT FAILED TO SIGN THE ACFT FLT MAINT LOGBOOK CORRECTIVE ACTION BLOCK AS 'TEST FLT SATISFACTORY' AND MAINT PERSONNEL FAILED TO SIGN OFF MAINT SIGNOFF BLOCK. THE ACFT FLEW 14 REVENUE FLTS WITH THE OPEN WRITE-UP; WHICH WAS NOT LISTED ON THE DEFERRED DISCREPANCY LOG AND COULD ONLY BE FOUND BY FLIPPING BACK 3 ACFT FLT MAINT LOGBOOK PAGES. I FLEW 1 OF THE 14 REVENUE FLTS (THE LAST BEFORE THE DISCREPANCY WAS DISCOVERED). THIS MISTAKE OCCURRED FOR 2 REASONS: LACK OF OVERSIGHT BY MAINT PERSONNEL AND THE CAPT PERFORMING THE TEST FLT FOR NOT ENSURING THE OPEN WRITE-UP WAS CLOSED BEFORE RELEASING THE ACFT TO THE LINE; AND FOR THE CAPTS WHO FLEW THE ACFT AFTER IT RETURNED TO THE LINE; THE ERROR SHOULD HAVE BEEN CAUGHT. I RECEIVED THE ACFT 15 MINS LATE; AND IN AN ATTEMPT TO GET BACK ON SCHEDULE; I TRUSTED THE PREVIOUS CAPT'S REVIEW OF PAPERWORK AFTER PERFORMING THE STANDARD PAPERWORK CHK OUTLINED IN MY COMPANY'S GENERAL OPS MANUAL. A NEW TEST FLT WAS PERFORMED TO CLOSE THE OPEN WRITE-UP AND THE ACFT WAS RETURNED TO THE LINE AFTER ALL PAPERWORK WAS COMPLETED AND XCHKED.
