Synopsis

AN MD-11 TAXIING FOR TKOF HAS #2 ENG FLAME OUT. SECOND INCIDENT FOR THIS ACFT.

Narrative

TAXIING FOR TKOF AT ZZZ THE NUMBER 2 ENG FLAMED OUT. THIS WAS THE SECOND OCCURRENCE IN 3 DAYS ON THAT AIRPLANE AND HAD IT OCCURRED ONLY MOMENTS LATER (<IMIN) WE WOULD HAVE BEEN ON TKOF ROLL. OUR MAINT DEPARTMENT WAS AWARE OF THE PROB AND; IN FACT; WAS IN POSSESSION OF A SVC BULLETIN FROM ACFT MANUFACTURE CO. TO CORRECT THE PROB. THIS SIT CONSTITUTES A HAZARD THAT SHOULD HAVE BEEN CORRECTED BEFORE RELEASING THE ACFT FOR SVC. AN ENG AUTHORIZATION ALLOWS MAINT TO RESTART THE #2 ENG AND DO A POWER ASSURANCE RUN-UP AND THEN RELEASE THE ACFT FOR SVC. APPARENTLY AIR SEEPS INTO THE FUEL LINES AND CREATES AN AIR GAP IN THE FUEL LINE SUPPLYING THE #2 ENG IF ACFT SITS FOR 8 HRS OR MORE. AFTER RESTARTING ENG AND RUNNING IT TO ELIMINATE AIR IN FUEL LINE; ACFT IS DEEMED SERVICEABLE. MY CONTENTION IS THAT AN ENG FLAME OUT HAS ALREADY OCCURRED (AND POSSIBLY AT A CRITICAL STAGE OF FLT); AND THE CAUSE SHOULD BE ADDRESSED; NOT THE SYMPTOMS; THE SIT SHOULD ALSO BE COMMUNICATED TO THE PLTS FLYING MD-11 ACFT IF THIS IS INDEED A PROB. ACFT MANUFACTURER CO. HAS IDENTIFIED THE PROB WITH THE AIRPLANE. IN 10 YRS OF FLYING THE MD-11; I HAVE NEVER ENCOUNTERED THIS SIT BEFORE ALTHOUGH I'VE FLOWN LOTS OF AIRPLANES THAT HAVE SAT FOR 8 HRS BTWN TURNS. I HAVE SEEN NOTHING IN WRITING ABOUT THIS PROB. FROM ANY SOURCE (UNTIL TODAY WHEN MAINT SHOWED ME A COPY OF THE ENGINEERING ORDER) CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE AIRPLANE WAS MOMENTS AWAY FROM TKOF WHEN THE #2 ENG FLAMED OUT. THE RPTR SAID THE AIRPLANE WAS RETURNED TO THE GATE WHERE MAINT MADE A POWER ASSURANCE RUN ON #2 ENG AND CLRED THE RPT REFERENCING A MAINT MANUAL PROCEDURE. THE RPTR STATED MAINT ALSO ADVISED THE CREW OF A MANUFACTURERS SVC BULLETIN DESCRIBING THE #2 ENG FLAME OUT WHEN PARKED FOR 8 HRS OR MORE. THE RPTR SAID THE SVC BULLETIN ALSO DESCRIBES THE REPAIR REQUIRED TO CORRECT THE SIT; BUT THE CARRIER HAS DECIDED TO WAIT UNTIL A HEAVY MAINT CHK IS DUE THEN ACCOMPLISH THE MODIFICATION. THE RPTR STATED NONE OF THIS INFO WAS EVER GIVEN TO THE MD-11 FLT CREWS.
