Synopsis

B737-300 CREW DID NOT COMPLY WITH A STAR XING RESTR AND HAD A SPD AND TRACK DEV ARRIVING AT EWR.

Narrative

CLRED TO DSND VIA THE DYLIN 1 ARR; I FAILED TO MEET THE XING RESTR AT DYLIN INTXN OF 8000 FT. THE PROB AROSE BECAUSE I DID NOT HAVE A CLRLY BRIEFED; CONSISTENT PLAN TO EXECUTE THE DSCNT. SPECIFICALLY; I BEGAN THE ARR IN VNAV; THEN SWITCHED TO A VERT SPD DSCNT WITHOUT ANNUNCIATING THE CHANGE TO THE CAPT. ALSO; I FAILED TO SET THE 8000 FT XING RESTR IN THE ALT WINDOW OF THE MCP UNTIL IT WAS TOO LATE TO MAKE THE RESTR. DSNDING FROM SOMTO INTXN AT 11000 FT MSL; I ATTEMPTED TO SLOW TO 250 KIAS PASSING THROUGH 10000 FT MSL; ALTHOUGH THAT SPD WAS NOT ATTAINED UNTIL 9600 FT MSL; APPROX THE ALT AT WHICH WE CROSSED DYLIN INTXN. THE CAPT DISCOVERED THE MISSED XING RESTR; AND TOLD ATC WE HAD MISSED IT. ATC SAID TO EXPEDITE TO 6000 FT; WITH NO RPTED TFC CONFLICT. STRESS AND RESULTING DEGRADATION IN HUMAN PERFORMANCE CONTRIBUTED TO LATE CAPTURE OF LOC FOR RWY 22L AT EWR; WITH ACFT FLYING THROUGH LOC; THEN CORRECTING BACK TO THE W TO CAPTURE IT. TO PREVENT THIS KIND OF MISTAKE; THE PF MUST THOROUGHLY BRIEF HIS PLAN OF FMC/FMS MODE CTL; AND BE PREPARED FOR ALL RESTRS (IN VNAV; SET LOWEST ALT RESTR; AS APPROPRIATE; ON THE ARR). THEN THE PLT MUST FLY HIS PLAN.
