H25A FLC INCORRECTLY PROGRAMS UNS AFTER RWY CHANGE AND DEVIATES FROM STAR.

Narrative

DSCNT INTO LAS VEGAS; ZLA ADVISED US TO PROCEED ON TRAGR 1 ARR; WINDS AT LAS VEGAS 140 DEGS AT 10 KTS. RWY 19L&R AND RWY 25L IN USE. ZLA HANDED US OFF TO LAS APCH AND THE FIRST CTLR SAID TO PLAN RWY 19R. UNS SYS WAS PROGRAMMED FOR RWY 19R PER 'PLAN 19R;' STAR RTE WAS VERIFIED AND ALT CHKED. APCH CTL THEN HANDED US OFF TO A SECOND CTLR. THIS CTLR SAID PLAN RWY 25L AND HE WOULD TRY FOR RWY 19R. AT THIS TIME WE WERE INSIDE FUZZY GOING TO TRAGR. HE ALSO ADVISED TO DSND VIA STAR. I BEGAN TO TRY TO REPROGRAM (UNS 1) SINCE IT HAS THE FULL DATABASE. UNS 2 IS AN OLDER VERSION AND LACKS ALL WAYPOINTS UNTIL XLOADED. I PRESSED THE MENU BUTTON WHICH WOULD GIVE ARR; BUT I FAILED TO REALIZE I DIDN'T SELECT FLT PLAN PAGE SO IT DIDN'T OFFER THE ARR OR RWY. SINCE THINGS WERE GOING FAST; I ASKED THE CAPT TO ENTER THE NEW RWY SINCE I WAS HAVING TROUBLE WITH THE SYS AND DIDN'T WANT TO WASTE TIME TRYING TO SET IT UP. THE CAPT GAVE ME CTL OF ACFT AND BEGAN PROGRAMMING THE SYS. HE MISTAKENLY PROGRAMMED RWY 25R WHICH COULD HAVE COME FROM HEARING '25L; BUT WILL TRY FOR 19R' BY CTLR. AFTER ENTERING INFO HE RESUMED CTL OF THE ACFT AND; SINCE THE AUTOPLT WAS ENGAGED; THE ACFT BEGAN TO FOLLOW THE NEW RTE. THE CAPT ASKED THAT I VERIFY THE RTE ON THE STAR PLATE. I SAW GEVOW WAS WHERE WE WERE NOW GOING SO I STARTED TO CHK THE PLATE. AT THIS TIME THE CTLR ASKED WHERE WE WERE GOING. I TOLD HIM GEVOW AND HE SAID WE WERE TO GO TO RWY 25L AND BEGAN TO ISSUE CONSECUTIVE L AND R TURNS AND A NEW ALT. UPON ARR AT LAS WE WERE ASKED TO CALL TRACON. THE CAPT CALLED AND WAS ADVISED THAT RWY 25L IS THE DEFAULT AND IT SHOULD BE PROGRAMMED UNTIL ADVISED BY ATC. WE BOTH LOOKED AGAIN AT THE STAR AND THEN NOTICED A NOTE SAYING TO PLAN RWY 25L UNLESS ADVISED. THINGS TO CORRECT: 1) VERIFY WITH APCH WHEN TOLD TO PLAN A RWY. 2) I NEED TO CHK WHICH PAGE UNS IS IN WHEN TRYING TO PROGRAM NEW RTE. IT SHOULD BE IN 'FLT PLAN'. 3) VERIFY WITH PERSON THAT DOES THE PROGRAMMING THAT THE CORRECT RWY IS ENTERED. 4) ATC NEEDS TO ASSIGN THE RWY ON A COMPLICATED STAR SOONER; SECOND CTLR MAY NOT HAVE KNOWN THAT THE FIRST CTLR SAID TO PLAN RWY 19R. 5) ALLOW MORE TIME IF A CHANGE IS MADE SO THAT THE CREW CAN VERIFY THE RTE.
