Synopsis

A B737-300 WAS OPERATED IN NON COMPLIANCE WITH THE R ENG FIRE EXTINGUISHING LINE DISCONNECTED AND CAPPED.

Narrative

ON DEC/MON/02; I RECEIVED DISCIPLINARY ACTION AND A LETTER IN MY FILE FOR UNSATISFACTORY JOB PERFORMANCE. HERE ARE THE CONTENTS OF THAT RPT: 'ON SEP/TUE/00 YOU SIGNED FOR WORK AS BEING ACCOMPLISHED ON ACFT WHICH WAS IN ZZZ BASE FOR A 'Q' CHK. COMPANY RECORDS INDICATE THAT YOU WERE THE MECH THAT ACCOMPLISHED THE INSTALLATION OF A R WING INBOARD FLAP OUTBOARD XMISSION. A PORTION OF THE INSTALLATION PROC WAS TO INSTALL A FIRE EXTINGUISHER AGENT LINE. ACFT RECENTLY WAS INDUCTED INTO MAINT FOR A SCHEDULED 'C' CHK. UPON INSPECTION AREA THAT YOU HAD PREVIOUSLY WORKED; IT WAS DISCOVERED THAT THE FIRE EXTINGUISHER LINE WAS DISCONNECTED AND CAPPED. FURTHER INVESTIGATION HAS SHOWN THAT NO ADDITIONAL WORK HAS BEEN ACCOMPLISHED IN THAT AREA SINCE LAST 'Q' CHK.' MY TIME SPENT IN THAT BASE MAINT FACILITY WAS APPROX 2 YRS. IN THAT SPAN; I CHANGED MANY FLAP XMISSIONS; FLAP TRACKS AND FLAPS. ALL IN ACCORDANCE WITH ACR X PAPERWORK -- WHICH AT THE TIME WAS VERY POOR. IT WAS A NEW FACILITY JUST MANNED WITH MANY NEW HIRES; JUST UPGRADED LEADS AND INSPECTORS; NOT TO MENTION WITH NEW INEXPERIENCED FOREMAN AND MGRS. THE PAPERWORK AND JOB CARDS AT THIS TIME WERE POOR AT BEST. BEING PERFORMED DURING THIS PERIOD WERE ALSO FLAP TRACK INSTALLATION AND ENGINEERING ORDERS BEING PERFORMED WHERE THE PAPERWORK WAS VERY CONFUSING. NEEDLESS TO SAY; I PERFORMED THE WORK; THEREFORE; I AM RESPONSIBLE. I HAVE YET TO SEE THE JOB CARD OR ENGINEERING ORDER THAT WERE SIGNED. THEREFORE; MY RECOLLECTION ON THAT PARTICULAR ACFT AND FLAP XMISSION ARE A BLUR WITH ALL THE OTHERS I ACCOMPLISHED.
