Synopsis

A B737-800 WAS DEICED WITH TYPE 4 FLUID AND WAS OPERATED IN NON COMPLIANCE AT SPDS EXCEEDING THE LIMITS STATED IN AIRWORTHINESS DIRECTIVE 2002-08-20.

Narrative

A TEMPORARY ACFT BULLETIN ON THE B737-800 REQUIRES A 270 KT AIRSPD RESTR AFTER THE APPLICATION OF DEICING FLUIDS TO THE ACFT; EXCEPT IN THE CASE OF TYPE 1 FLUID. THE RESTR CAN BE LIFTED AFTER A MAINT INSPECTION; OR PERMANENTLY LIFTED IF NA AIRFRAME MODIFICATION HAS BEEN COMPLETED. TO DATE; THIS MODIFICATION HAS BEEN COMPLETED ON MANY OF OUR B737-800'S. IT WAS SUPPOSED TO BE MY LEG; BUT BECAUSE I HAVE LESS THAN 100 HRS ON THIS ACFT; COMPANY POLICY REQUIRES THE CAPT TO DO TKOFS AND LNDGS UNDER CERTAIN OPERATIONAL SITS. JUST PRIOR TO PUSHBACK; CONDITIONS SEEMED TO PERMIT ME TO MAKE THE TKOF; BUT VERY QUICKLY DETERIORATED DUE TO SNOW WHICH ALSO NECESSITATED OUR DEICING. AT FIRST; DEICING PERSONNEL MENTIONED THEY WOULD BE USING TYPE 1 FLUID AND SOMEWHERE IN THE SHUFFLE WE SWITCHED TO TYPE 4 FLUID INSTEAD. OUR HASTE TO REMAIN WITHIN OUR DETERMINED DEICING HOLDOVER TIME; ADJUSTING OUR TKOF NUMBERS IN THE FMC; HAVING TO START BOTH ENGS WHEN DEICING WAS COMPLETED; AND COMPLY WITH THE EXTRA PROCS ASSOCIATED WITH DEICING; COMPLETION OF ALL OUR REQUIRED CHKLISTS (DE-/ANTI-ICING; PUSHBACK/START; AFTER START; TAXI; BEFORE TKOF); HAVING THE CAPT REBRIEF THE TKOF BECAUSE HE WOULD NOW BE FLYING IT; COMPLYING WITH SOMEWHAT COMPLEX TAXI ROUTING TO RWY AT AN ARPT THAT I'VE NEVER FLOWN INTO BEFORE AND IS NOT NORMALLY SERVED BY THE B737-800. (WE WERE ON AN EQUIP SUBSTITUTION FOR AN MD88); ALSO A LAST MIN REQUEST BY TWR TO CHANGE DEP RWY (WE DECLINED). THESE ALL COMBINED TO CREATE A SIT WHERE THE TEMPORARY ACFT BULLETIN 270 KT AIRSPD RESTR WAS OVERLOOKED. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED AT THE LAST MIN THE GND CREW ADVISED THE FLT CREW THAT TYPE 4 DEICING FLUID WOULD BE USED INSTEAD OF TYPE 1 AND THAT WOULD REQUIRE A SPD RESTR. THE RPTR SAID PER THE COMPANY BULLETIN ON DEICING FLUIDS IF TYPE 1 IS USED NO SPD RESTR APPLIES. THE RPTR STATED VARIOUS PROCS AND CHKLISTS WERE ACCOMPLISHED AND CREATED A SIT WHERE THE DEICING BULLETIN AND SPD RESTR WAS OVERLOOKED. THE RPTR SAID THE TYPE 1 FLUID WAS STILL IN OUR MIND WITH NO SPD RESTR. THE RPTR WAS ADVISED THAT ACCORDING TO THE AIRWORTHINESS DIRECTIVE THE AIRPLANE IS SPD RESTR WHEN ANY GND DEICING/ANTI-ICING OF THE HORIZ STABILIZER OCCURS.
