INPUTS TO THE FMS AFFECTING BOTH PLTS WERE MADE WITHOUT MONITORING OR VERIFICATION. A CONFLICT WITH AN ACR B737 CORRECTLY ON COURSE TO PARALLEL RWY 22L RESULTED.

Narrative

ON APCH TO ORD FROM S WITH LATE RWY ASSIGNMENT TO ILS RWY 22R. AS PF; I HAD PLANNED FOR AND SELECTED VISUAL RWY 22L IN FMS. UPON RECEIVING THE ASSIGNMENT FOR ILS RWY 22R; I CHANGED FREQ AND COURSE FOR RWY 22R; BUT FORGOT TO CHANGE THE FMS FOR RWY 22R. FMS SHOULD GENERATE A FREQ MISMATCH MESSAGE ON THE FLT DISPLAY BUT DID NOT IN THIS CASE. CTLR WAS VERY BUSY AND HAD US ON A CLOSE PARALLEL COURSE; WHICH RESULTED IN A 'TIGHT' TURN TO FINAL. DURING THE TURN TO FINAL; I WAS IN MAP MODE WHILE CAPT WAS IN RAW DATA. BECAUSE I HAD NOT CHANGED THE FMS FROM RWY 22L TO ILS RWY 22R; MY MAP DISPLAY SHOWED ME ABOUT TO OVERLY FINAL. SUBSEQUENTLY; THE CTLR REQUESTED US TO TIGHTEN OUR TURN. 3 THINGS HAPPENED AT THIS TIME TO COMPLICATE THE SIT: 1) CTLR REQUEST TO TIGHTEN TURN TO 240 DEGS; 2) TA FROM ACFT N OF COURSE BEING VECTORED TO FINAL; 3) FMS MAP PRESENTATION SHOWING OVERSHOOT. IN REALITY; WE HAD NOT YET REACHED RWY 22R LOC/CTRLINE. BECAUSE ILS RWY 22R HAS NO DME; CAPT HAD BEEN MONITORING ORD VOR UNTIL OUR TURN TO BASE. HE THEN SWITCHED TO ILS RWY 22R TO MONITOR RAW DATA FOR LOC INTERCEPT AND WAS ABLE TO MAKE THE CALL TO ME THAT WE WERE STILL S OF LOC AND TO CORRECT BACK TO THE N. AT THIS TIME THE CAPT VISUALLY ACQUIRED BOTH RWYS 22L AND 22R AND WE CONTINUED OUR CONNECTION TO RWY 22R CTRLINE. DURING OUR MANEUVER CORRECTING TO CTRLINE; ACR FLT QUESTIONED THE CTLR ABOUT OUR FLT PATH AND SPACING FOR LNDG. TO OUR KNOWLEDGE; NO EVASIVE OR ABNORMAL MANEUVERING WAS REQUIRED. AT ABOUT 3-4 MI ON FINAL; WE REALIZED THE FMS MISTAKE. LNDG WAS UNEVENTFUL AND WE DEBRIEFED THE EVENT. FACTORS: LATE RWY ASSIGNMENT. NO DME FOR ILS RWY 22R. TIGHT TURN TO FINAL FROM S WITH ACFT TURNING BASE N OFF COURSE RESULTING IN TA. NO FREQ MISS-MATCH MESSAGE ON FMS. WRONG RWY ASSIGNMENT IN FMS.
