Synopsis

A B767-200 FLT CREW DSNDS 300 FT BELOW MDA ON THE VOR RWY 13L APCH INTO JFK.

Narrative

VECTORED FOR A SECOND VOR RWY 13L AFTER MISSED APCH ON RWY 13L DUE TO HIGH WINDS AND LACK OF VISIBILITY. 1ST APCH VECTORED INTO CRI (FAF) AT 1500 FT MSL WHICH LEFT 700 FT DOWN TO DA (MDA). 2ND APCH VECTORED IN AT 1000 FT MSL; WHICH IS MINIMUM VECTORING ALT FOR ATC; INTO CRI WHICH LEFT ONLY 200 FT DOWN TO DA (MDA). WE WERE BRIEFED UP FOR THE 1500 FT INTERCEPT FROM THE ORIGINAL BRIEF. WE STARTED DOWN AT CRI WITH 700 FPM RATE OF DSCNT WHICH WORKED WELL FOR APCH NUMBER ONE; BUT NOT APCH NUMBER TWO. BOTTOM LINE WAS THAT WE DSNDED APPROX 150 FT BELOW DA (MDA) AND STARTED AN IMMEDIATE CORRECTION WHEN JFK TWR GAVE US A LOW ALT ADVISORY. WE WERE USING CONSTANT DSCNT APCH PROC. ALONG WITH THE HIGH WINDS; HVY RAIN; AND COMING OFF AN ALL NIGHTER; ITEMS THAT MADE IT ALL MORE DIFFICULT. WE WERE IN VMC CONDITIONS BELOW 1200 FT MSL; BUT STILL ON AN INST APCH. REMAINDER OF THE APCH AND LNDG WAS NORMAL. I'M FRUSTRATED THAT MY FATIGUE LEVEL WAS A FACTOR; I DO PRIDE MYSELF IN MY USE OF SOP'S AND MY TRAINING. ANY SUGGESTIONS WOULD BE HELPFUL.
