Synopsis

ACR CREW TRANSPORTED DANGEROUS GOODS THAT WERE NOT ALLOWED; AND IN A PIT LOCATION THAT IS PROHIBITED FOR DANGEROUS GOODS.

Narrative

UPON ARR TO THE ACFT IN DLH; A STATION AGENT CAME UP THE JETWAY STAIRS AND PRESENTED ME WITH 1) A DEICING NOTIFICATION FORM; 2) THE FUEL SLIP; AND 3) THE ORIGINATION FLT SECURITY FORM. UPON PRESENTING THESE PAPERS TO ME; HE SAID THERE MIGHT BE A #1 FORM ON MY YOKE CLIP. THINKING HE MISSPOKE; I ASKED WHETHER IT WAS A #1 FORM OR A DEICING NOTIFICATION FORM; SINCE HE HAD THAT ONE IN HIS HAND. HE REPLIED THAT THE DE-ICING NOTIFICATION FORM HE BROUGHT UP WAS A COPY SINCE HE WASN'T SURE WE HAD ONE IN THE COCKPIT. THERE WAS NO FURTHER MENTION OF A #1 FORM; NOR DID I EVER SEE ONE. UPON ARR IN ZZZ AND DURING MY POSTFLT; THERE WERE 2 GND AGENTS AND A SUPVR LOOKING INTO CARGO BIN #1 (A BIN NOT APPROVED FOR DANGEROUS GOODS). I ALSO LOOKED INTO THE BIN AND SAW A BOX WITH A DANGEROUS GOODS CLASSIFICATION LABEL. I SPOKE BRIEFLY WITH THE SUPVR WHO CONFIRMED THAT WE HAD CARRIED NON-ALLOWABLE DANGEROUS GOODS IN A PROHIBITED BIN AND HE SHOWED ME THE UNSIGNED #1 FORM. A DISPATCH SUPVR AND CHIEF PLT WERE NOTIFIED IN A CONFERENCE CALL ONCE I GOT TO A PHONE. I BELIEVE I MADE A REASONABLE ATTEMPT TO CLARIFY THE STATION AGENT'S STATEMENT REGARDING THE POSSIBLE #1 FORM. HOWEVER; GIVEN THE IMPORTANCE OF PROPER HANDLING OF DANGEROUS GOODS; I WOULD IN THE FUTURE TAKE EXTRA TIME TO MAKE CERTAIN WHETHER OR NOT A STATION IS CONTEMPLATING A DANGEROUS GOODS SHIPMENT. WHEN I LOOKED OVER THE UNSIGNED #1 FORM UPON ARR IN ZZZ; THE FORM VERY CLRLY DESCRIBED A DANGEROUS GOODS THAT I KNOW WE ARE NOT ALLOWED TO CARRY. HAD I SEEN THE FORM; THE SHIPMENT WOULD NOT HAVE GOTTEN ON BOARD THE ACFT.
