Narrative
ENTERING FINAL SEGMENT; IN LEFT TURN TO DEP HDG; R T/R 'REVERSER UNLOCK' ANNUNCIATED. EXECUTED 'REVERSER DEPLOY IN FLT' CHKLIST. IN VMC; ADVISED ZZZ TWR OF SIT AND LEVELED AT 2000 FT (CLRED TO 4000 FT). TWR ADVISED 'MAINTAIN VFR' HANDOFF TO ZZZ DEP FOR VECTORS TO VISUAL APCH TO ZZZ1. UNEVENTFUL LNDG; NO SPECIAL HANDLING WAS REQUESTED; NOTIFIED COMPANY. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE REVERSER SLEEVE MUST HAVE MOVED SLIGHTLY AFT AS THE BUFFETING AND VIBRATIONS WERE STRONGLY FELT THROUGHOUT THE ACFT. THE RPTR STATED ACTIVATING THE EMER STOW STOPPED THE BUFFETING AND VIBRATIONS AND PUT OUT THE UNLOCK WARNING LIGHT. THE RPTR STATED THE REVERSER DID NOT FULLY DEPLOY AS THE GREEN REVERSER DEPLOY LIGHT NEVER ILLUMINATED; BUT IT CERTAINLY DID UNLATCH. THE RPTR SAID THE AIRPLANE LANDED WITH BOTH REVERSERS INOP AND NEEDED HEAVY BRAKING. AFTER PARKING; THE #4 MAIN TIRE FUSE PLUG FAILED FROM OVERHEATING AND CAUSED THE TIRE TO GO FLAT. THE RPTR SAID THE AIRPLANE WAS TAKEN TO A MAINT FACILITY FOR HEAVY MAINT AND REVERSER REPAIR AND REMAINED THERE FOR ALMOST 4 WEEKS. THE RPTR STATED THE FOLLOWING MAINT AND CORRECTIVE ACTION WAS ACCOMPLISHED ON THE RIGHT THRUST REVERSER. THE RIGHT THRUST REVERSER HARNESS WAS REPLACED PER THE MAINT MANUAL. READJUSTED MAIN TRACK SLIDER BLOCKS PER MAINT MANUAL. READJUSTED PNEUMATIC DRIVE MAINT MANUAL. READJUSTED BALL SCREW ACTUATORS PER MAINT MANUAL. RIGGED THRUST REVERSER AS PER MAINT MANUAL. OPERATIONAL CHECK AND FLT TEST OK. DEFERRED ITEM MEL CLEARED.
