AN A300 WAS OPERATED WITH 2 PYLON FASTENERS MISSING WITH NO MEL REFERENCES OR CONFIGURATION DEV LIST REF. LOGBOOK RPT WAS CLRED USING A REPAIR DOCUMENT REFERENCE THAT EXCLUDED PYLON FASTENERS.

Narrative

THIS EVENT OCCURRED THE MORNING OF 9/SUN/05; ON FLT X; A LEG FROM ZZZ1 TO ZZZ2. THIS WAS AN AIRBUS FLT; ACFT X. CAPT HAD FLOWN THIS ACFT ON 9/THU/05; AND HAD A DISCREPANCY WITH MISSING SCREWS THAT SECURE METAL COVERINGS TO THE ENGINE PYLON. HE NOTED THIS IN THE ACFT LOG BOOK. MAINT REVIEWED AND SIGNED THE ACFT MAINT LOG THAT THE ITEM WAS OK FOR FLT. CAPT THEN QUESTIONED THE REPAIR AND SIGN OFF IN THE MAINT BOOK. THE CHIEF PLT WAS CALLED IN AND THE CAPT WAS TOLD THE ACFT SIGNOFF WAS GOOD. THE PROB WOULD BE FIXED. NOW IT IS 9/SUN/05 AND THE CAPT GETS THE SAME AIRPLANE AGAIN TO FLY AND THE SCREWS ARE STILL MISSING. NO REPAIRS MADE. HE WRITES-UP THE ITEM AGAIN. THEY CLEAR THE ITEM AGAIN WITH NO REPAIR OR TAPE OR SCREW TO REPLACE THE MISSING ONE. THEY USE A MAINT REF # X-SRM-51-00-00-1 ALSO SRM-51-10-12-0 TO CLR THE ITEM. THERE IS NO MEL OR CONFIGURATION DEVIATION LIST TO COVER THIS MISSING SCREW. THERE IS NO ENTRY IN THE MAINT ITEM CTL SHEET TO SAY IF THIS WERE AN OPEN ITEM; AND IT NEEDED TO BE FIXED. THE CAPT CALLS THE CHIEF PLT AND WE ARE TO FLY THE ACFT. THIS WAS MY FIRST TIME FLYING THE PLANE. THIS WAS THE CAPT'S 2ND TIME. THAT THE ITEMS WERE WRITTEN UP ON AND THE NOTE THE CHIEF SENT TO US TO FLY THE ACFT. I FEEL WITH NO MEL OR CONFIGURATION DEVIATION LIST TO COVER THE MISSING PART; THE MAINT SIGNOFF WAS WRONG. WITH NO REPAIR FOR THE SCREW THAT WAS MISSING WAS WRONG. MY CHIEF PLT WAS ALSO WRONG. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE LOGBOOK WRITE-UP ON THE MISSING FASTENERS WAS CLRED USING THE REPAIR DOCUMENT AS A REFERENCE THAT EXCLUDED PYLON PANEL FAIRING AND PANELS. THE RPTR EXPERIENCED THIS CLRING OF THE MISSING FASTENERS LOGBOOK RPT ON ONE FLT BUT STATES THE CAPT EXPERIENCED TWO FLTS WHERE THIS MISSING FASTENER LOGBOOK RPT WAS CLRED WITH THE REPAIR DOCUMENT. SUPPLEMENTAL INFO FROM ACN 671452: REQUESTING ENGINEERING REVIEW AS REQUIRED BY MAINT PROC. NO ACTION BY ENGINEERING. ACFT FLOWN WITH WRITE-UP AUTHORIZING DISPATCH; NO ENGINEERING APPROVAL. CALLBACK CONVERSATION WITH RPTR ACN 671452 REVEALED THE FOLLOWING INFO: THE RPTR STATED THE LOGBOOK MISSING FASTENER RPT WAS CLRED ON TWO SEPARATE FLTS ON DIFFERENT DAYS USING THE REPAIR DOCUMENT. HE COULD GET NO EXPLANATION WHY THE REPAIR DOCUMENT WAS CLRLY NOT ADDRESSING THE PYLON MISSING FASTENERS AND EVEN EXCLUDED PYLON PANEL AND FAIRINGS. MAINT CTL WAS CONTACTED AND WOULD NOT ALLOW ACCESS TO A STRUCTURES ENGINEER TO DETERMINE IF WRITTEN ENGINEERING APPROVAL WOULD BE ALLOWED. THIS AIRPLANE WAS FLOWN WITH THE MISSING FASTENERS WITH NO OPEN LOG ITEM OR REFERENCE FOR DEFERRAL FROM THE MINIMUM EQUIPMENT LIST CONFIGURATION DEVIATION LIST OR ANY WRITTEN APPROVAL FROM STRUCTURES ENGINEERING. THE CHIEF PLT WAS CONTACTED AND BRIEFED ON THE MAINT PROB AND RESULTED IN THE CREW BEING DIRECTED TO FLY THE AIRPLANE.
