Synopsis

CARJ FLT CREW RPTS LOSS OF TRANSITION WAYPOINTS AFTER EXECUTING DIRECT NOBLY CLRNC ON THE NOBLY RNAV SID FROM DFW.

Narrative

CLRED AS FILED NOBLY2; LIT TRANSITION ENRTE TO CLE. FLT PLAN WAS ENTERED INTO THE FMS AND VERIFIED BETWEEN CREW MEMBERS AS PER FLT MANUAL AND ACFT OPERATING MANUAL PROCS. ALL WAYPOINTS VERIFIED TO FLT PLAN; CLRNC AND DEP/ENRTE CHARTS. DEPARTED NORMALLY. DURING THE RNAV DEP; ATC CLRED US DIRECT FROM PRESENT POSITION TO NOBLY INTXN; AND THE REST OF THE DEP PLAN. PNF ENTERED THE INFO INTO THE FMS; STATED 'CONFIRM' TO THE PNF. PNF VERIFIED THE INFO ENTERED INTO THE FMS AND STATED; 'EXECUTE.' NAV MODE WAS STILL ACTIVE; AND THE NEW COURSE WAS ESTABLISHED. ATC SOON CONTACTED US TO VERIFY IF WE WERE INDEED ON THE DEP. WE BECAME AWARE THAT THE WAYPOINTS COBUG AND ORTRO WERE NO LONGER IN OUR LEGS PAGE RESULTING IN A COURSE OF DIRECT LIT. WE WERE CLRED TEN DEGS TO THE R AND TO CONTACT THE NEXT CTR FREQ. THE NEXT CTR CTLR CLRED US DIRECT TO ORTRO; REST OF RTE AND NORMAL COURSE WAS SHORTLY REESTABLISHED WITHOUT FURTHER INCIDENT. ALL COMPANY PROCS IN PROGRAMMING THE FLT PLAN; VERIFYING THE RTE; DEPARTING WITH THE CORRECT DEP IN THE FMS AS VERIFIED BY DFW QUEUE OPERATIONS WERE COMPLIED WITH. BOTH PLTS VERIFIED THE NOBLY CLRNC AS PER AOM. WE DO NOT KNOW HOW COBUG AND NOBLY WERE SOMEHOW NO LONGER IN THE FMS. WE BOTH HAVE HEARD OF THIS HAPPENING TO OTHER CREWS; BUT NEVER EXPERIENCED IT OURSELVES UNTIL TODAY. ATC DID NOT INFORM US OF ANY VIOLATION; AND THE FLT CONTINUED TO DEST WITHOUT ANY INCIDENT. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR WAS ADAMANT ABOUT THE PROPER INSTALLATION AND ACTIVATION OF THE DEP AND TRANSITION AS WELL AS THE VERIFICATION OF ALL WAYPOINTS PRIOR TO DEP. HE FURTHER ADVISED THAT HIS CAPT ON THIS FLT WAS THE ACR CARJ FLEET CAPT WHO WAS PARTICULARLY SENSITIVE TO THE RECURRING SIT AND WAS; THUS; MORE INSISTENT THAN NORMAL THAT ALL PROPER COMPANY PROCS BE ADHERED TO. ON A SUBSEQUENT FLT; RPTR ATTEMPTED TO RECREATE THE ANOMALY BY BUILDING A WAYPOINT PRIOR TO NOBLY AND -- AS THEY APCHED IT -- SELECTING AND ACTIVATING A DIRECT TO NOBLY COMMAND. AFTER DOING SO; THE TRANSITION WAYPOINTS DID NOT DISAPPEAR AS THEY DID ON THE FLT DISCUSSED IN THE RPT.
