Narrative
I WAS WORKING MEAD DEP (MED) CONFIGN 3; I HEARD SOMEONE YELL ACFT X IS GOING AROUND LOOKED AT THE ARPT AND SAW ACFT X AND THE ARR TO RWY 1 MODE C 100 FT APART CAN'T REMEMBER IF CONFLICT ALERT ACTIVATED THEN TWR FLASHED ACFT X TO ME AT MED. I CALLED TWR AND SAID IT WAS DAGGATT DEP'S AIRSPACE AND THAT ACFT X SHOULD GO TO DAG. THIS WAS ABOUT 2 MI W OF DEP END OF RWY 25L. ACFT X WENT MISSED FROM RWY 25L. I AM 90% SURE ACFT X ENTERED THE 6000 FT MOA W OF LAS AT 5000 FT. I HAD TFC IN MY AIRSPACE AND WAS BUSY WITH WX DEVS. I COULD HEAR DAG YELLING IN THE BACKGROUND THAT THEY ARE NOT TALKING TO ACFT X; HE WAS NOT ON MY FREQ. SUPPLEMENTAL INFO FROM ACN 696345: BOTH ACFT #1 AND #2 WENT AROUND DUE TO WINDSHEAR. ACCORDING TO SOP THE ACFT WERE ASSIGNED 5000 FT; EVEN THOUGH THE MOA 6 NM W OF LAS IS 6000 FT THE DEP CTLR DID NOT HAVE RADIO CONTACT WITH EITHER ACFT BEFORE THEY ENTERED THE 6000 FT MVA. THIS EVENT WAS TOTALLY CAUSED BY A POOR PROC AND RESULTED IN AT LEAST THESE 2 ACFT TO BE ASSIGNED AN ALT BELOW THE MVA. ADDITIONALLY; LAS TWR WAS UNSURE WHO TO HAND OFF THE ACFT TO AND WHO TO FREQ CHANGE THEM TO EVEN THOUGH THEY WERE ADVISED BY THE L30 SUPVR. MGMNT WAS AWARE OF AND ADVISED OF THE SITUATION BUT AS FAR AS I KNOW NO CORRECTIVE ACTIONS HAVE OCCURRED.
