Synopsis

EMB145 CREW COMPRISED OF TWO CAPTS FAILS TO TURN PACKS OFF FOR TKOF IAW COMPANY PROC.

Narrative

I WAS THE PF ON THIS LEG. ON TKOF ROLL AT AROUND 120 KTS SMOKE FILLED THE COCKPIT. I REACHED UP AND TURNED OFF THE AIR CONDITIONING PACKS AND CONTINUED THE TKOF. THE EICAS MESSAGE READ 'LAV SMOKE.' AFTER CLEAN-UP; THE PNF (CAPT) PULLED OUT HIS AOM VOL 1 AND EXECUTED THE APPROPRIATE PROC. THE SMOKE DISSIPATED AND THE CAPT VERIFIED WITH THE FLT ATTENDANT THAT THE LAV WAS CLR OF SMOKE. FLT ATTENDANT CONFIRMED THE LAV WAS CLR. WE CONTINUED THE FLT WITHOUT FURTHER INCIDENT.CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE EMB WAS DESIGNED TO BE OPERATED WITH PACKS ON FOR TKOF; HOWEVER A DESIGN DEFICIENCY IN A SEAL ALLOWS ENG OIL INTO THE BLEED AIR SYSTEM DURING HIGH POWER SETTINGS. THIS IS MOST COMMON ON THE FIRST TKOF OF THE DAY. COMPANY PROC HAS BEEN CHANGED AND ALL TKOFS ARE PERFORMED WITH PACKS OFF TO PREVENT THIS PROB. THE CAPT QUALIFIED FO FAILED TO ACCOMPLISH THIS ITEM ON THE TKOF CHKLIST. THE RPTR STATED THEY QUICKLY REALIZED THEIR MISTAKE WHEN THE OIL FUMES CAME INTO THE COCKPIT AND THEY CORRECTED IT. THE ENG MANUFACTURER IS IN THE PROCESS OF REDESIGNING THE SEAL.
