Narrative

ACFT ACR X WAS DE-ICED WITH KNOWLEDGE OF THE DE-ICING PLAN REQUIREMENTS IN ACCORDANCE WITH DE-ICING PLAN PER FAR 121.39. DE-ICING OF ACR X WAS ACCOMPLISHED BY CONTRACTORS (USING COMPANY EQUIP AND FLUIDS) AND ANOTHER COMPANY (FBO USING EQUIP AND FLUIDS SUPPLIED FROM OTHER THAN OUR COMPANY). ACR X SCHEDULED FLT FLEW FROM TO ZZZ1. ACR X WAS INSPECTED BY THE CAPT OF THE FLT CREW AND ACFT MAINT AND DETERMINED TO BE CLEAN (FREE OF ANY ICE ACCUMULATION ON VISIBLE CRITICAL AREAS AND/OR THE REMAINING FUSELAGE). ACR X DEPARTED ZZZ JAN/MON/07 WITHOUT INCIDENT. INFO FROM THE FLT CREW UPON ARR AT THE ZZZ1 GATEWAY WAS COMMUNICATED TO THE DEP GATEWAY. ACFT MAINT THE FOLLOWING DAY. THAT INFO INCLUDED THAT ACR X HAD ACCUMULATIONS OF ICE ON THE ACFT IN CRITICAL AREAS AND THAT THE CAPT RPTED BUFFETING INTO ZZZ1 OF A NOTICEABLE NATURE AND/OR CONCERN. ACFT MAINT SAFETY CONCERNS SURROUND THE CONDITIONAL CHARACTERISTICS OF THE FLUID MIXTURES APPLIED TO ACR X AT THE DEPARTING GATEWAY. ACFT MAINT SAFETY CONCERNS SURROUND THE APPLICATION TECHNIQUES AND DE-ICING PLAN PROCS RECOGNITION INSTRUMENTED BY THE APPLYING SVCS.
