AN SMA PLT LANDED GEAR UP WHILE WEARING A NOISE CANCELLING HEADSET; SUGGESTING; WIRING THE GEAR WARNING INTO THE ACFT AUDIO SYSTEMS.
Narrative
WE HAD A GEAR UP LNDG. MY FAULT. SUGGESTION: WHEN WEARING NOISE CANCELLING EAR PHONES; THE GEAR WARNING SHOULD BE WIRED INTO THE AUDIO SYSTEM.CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THAT THE SEQUENCE OF EVENTS LEADING UP TO THE INCIDENT CAUSED HIM TO BECOME DISTRACTED. HE HAD JUST ENTERED THE VFR TFC PATTERN AND WAS ASKED BY ATC TO SLOW HIS ACFT AND THEN FOLLOW A SLOWER ACFT. THAT ATC CALL; HIS VERBAL RESPONSE; AND ACFT CTL REACTION OCCURRED AT THE POINT IN HIS NORMAL PATTERN FLOW WHERE HE WOULD HAVE LOWERED THE LNDG GEAR. HE SUBSEQUENTLY FORGOT TO GO BACK TO THE CHKLIST AND REVIEW IT BECAUSE HE WAS CONCENTRATING ON FLYING THE ACFT AT A REDUCED AIRSPD. HIS ACFT DOES NOT HAVE VISUAL GEAR INDICATOR LIGHTS IN ADDITION TO THE HORN BUT AGAIN HE WAS DISTRACTED AND SIMPLY FAILED TO REVIEW THE CHKLIST AND HIS NORMAL PROCESS. IN ADDITION TO WEARING THE NOISE CANCELLING HEADSET; HE DOES HAVE A HEARING LOSS THAT COMPLICATES HIS SITUATION. THE NOISE CANCELLING HEADSET IS WORN IN ORDER TO PROTECT HIS HEARING BUT AT THE SAME TIME IT MASKS NEEDED INFO.
