EMB145 FLT CREW REPORTS IMPROPER SIGNOFF FOR GALLEY DOOR WARNING FAILURE; WITH BOTH AN MEL NUMBER AND AN OPS CHECK OK ENTERED IN THE LOG.
Narrative
WE HAD BEEN RUNNING LATE THAT DAY AND WERE TRYING TO PLAY CATCH-UP. WE SWAPPED PLANES IN ZZZ FOR OUR FLT TO SDF. ON THE RELEASE; IT HAD AN MEL LISTED FOR THE GALLEY DOOR INDICATION. THE MEL WAS STILL OPEN IN THE DEFERRAL LOG. I THEN CHKED THE LOGBOOK PAGE WHERE IT HAD BEEN DEFERRED. IN THE FIRST CORRECTIVE ACTION BOX; MAINT HAD DEFERRED THE GALLEY CABIN DOOR INDICATION; PLACARDED THE MFD WITH A STICKER; WHICH I RELATED TO ALL OTHER SIGNS LEADING TO A CORRECT DEFERRAL. WE THEN CHKED THE MEL FOR THE PLT'S ACTION AND FOLLOWED OUR INSTRUCTIONS FOR THE MEL. HOWEVER; THE 2ND CORRECTIVE ACTION BOX HAD AN 'OPS CHK GOOD' ON THE GALLEY DOOR WHICH I DID NOT SEE. I WASN'T USED TO SEEING A DEFERRAL SIGNOFF AND AN 'OPS CHK GOOD' ON THE SAME LOGBOOK PAGE. WHEN WE LANDED IN SDF; I NOTICED THAT THERE WAS NO LONGER AN MEL LISTED ON THE RELEASE. I CALLED DISPATCH AND THEY TOLD ME THE MEL WAS CLRED IN ZZZ BEFORE WE HAD TAKEN OFF. IT WAS NOT PROPERLY SIGNED OFF AND I DID NOT CATCH IT. I SHOULD HAVE READ ALL THE CORRECTIVE ACTION BLOCKS ENTIRELY. THE MAINT GUY WHO ATTEMPTED TO CLR THE MEL WAS NEW AND DIDN'T FINISH THE PAPERWORK CORRECTLY. MAYBE WE SHOULD USE 2 DIFFERENT LOGBOOK PAGES; ONE FOR ACTUALLY DEFERRING THE ITEM; AND ANOTHER ONE FOR ACTUALLY FIXING THE PROB. IT SEEMS ODD TO DEFER THE EQUIP AND THEN OPS CHK IT GOOD 1 BOX DOWN ON THE SAME PAGE.
