A REASSIGNMENT; A SHORT LAYOVER; NO DINNER AND MINIMUM TIME TO PREPARE FOR THE DAY'S FIRST FLIGHT ALL CONTRIBUTE TO A TRACK DEV WHEN THE CLEARED ROUTE IS IMPROPERLY ENTERED INTO THE FMC.

Narrative

WE ARRIVED LATE TO THE AIRPLANE AFTER A LONG (30 MIN) VAN RIDE FROM THE HOTEL; AFTER A SHORT (8 HR 30 MIN BLOCK-IN TO SHOW TIME) OVERNIGHT. WE WERE RUSHED TO MAKE AN ON-TIME DEP. WE WERE CLRED AS FILED FROM LCH VIA LCH.V306.DAS6.... PRIOR TO PUSHBACK; THE CAPT BRIEFED THE DEP. HE BRIEFED THE AIRWAY COURSE FROM LCH CORRECTLY (265 DEG RADIAL). AFTER TKOF; WE WERE INSTRUCTED TO PROCEED ON COURSE. I SEQUENCED THE FLT PLAN IN THE FMS AND SET UP AN INTERCEPT HDG. UPON REACHING 10000 FT; LCH DEP ASKED IF WE WERE INTERCEPTING V306. I ASKED LCH TO SAY AGAIN. LCH REPLIED THAT WE WERE GOING N OF V306 AND THAT WE WERE NOW TO PROCEED DIRECT DAS. HERE'S WHAT HAPPENED: THE CAPT ENTERED THE FLT PLAN IN THE FMS PRIOR TO DEP BY ENTERING 'LCH;' THEN 'V306.DAS;' THEN SELECTING THE 'LCH.DAS6' ARR INTO ZZZ. HOWEVER; THE LCH.DAS6 TRANSITION PROCEEDS TO SURVE AFTER LCH; WHICH IS NOT ON; AND N OF; V306. ESSENTIALLY; V306 WAS REMOVED AND THE WRONG COURSE INSERTED IN ITS PLACE. I DID NOT XCHK THIS WITH THE PAPERWORK; AGAIN; BECAUSE I WAS TOO BUSY BEFORE DEP. THIS EXACT EVENT HAS HAPPENED BEFORE; AND WE HAVE BEEN WARNED ABOUT IT. IN HINDSIGHT; I WAS QUITE FATIGUED IN ADDITION TO BEING RUSHED. I HAD SHOWN THE DAY BEFORE EXPECTING TO FLY ONLY A DAY TRIP. I WAS EXTENDED FOR THE OVERNIGHT (I AM NOT ON RESERVE; AND THIS IS NOT COMMON PRACTICE). I FELT FINE THAT NIGHT; BUT I HAD ONLY 20 MINS BTWN FLTS; WHICH PREVENTED ME FROM OBTAINING DINNER. THAT; FOLLOWED BY A MINIMUM-REST OVERNIGHT; CAUGHT UP WITH ME THE NEXT MORNING.
