B737-800 FLT CREW FAILS TO NOTE REVISION TO FILED ROUTE WHEN PROGRAMMING THE FMS ROUTE.
Narrative
ON CLBING OUT OF CVG; WE WERE DEVIATING AROUND SOME WX. INDY CTR CLEARED US DIRECT IIU UPON COMPLETION OF THE DEVIATION. AFTER IIU CTR ASKED IF WE WERE HEADING DIRECT TO PXV. WE STATED WE WERE GOING TO MYS FPR. UPON FURTHER INVESTIGATION WE NOTICED THAT OUR CLRNC HAD BEEN CVG-IIU-PXV INSTEAD OF CVG-IIU-MYS-PXV; WHICH WE HAD IN THE FMS. WE HAD MISSED THE ROUTE CHANGE. THE SIMILARITY OF THE FLT PLAN TO ATC CLRNC WAS THE PROBLEM. SO CLOSE BETWEEN THE TWO AND MAYBE THE SHORT NIGHT BEFORE HAD CAUSED US BOTH TO MISS THE CLRNC. OUR DISPATCH HAS BEEN NOTIFIED AND A BULLETIN HAS BEEN ISSUED TO THE PLT GROUP. I HOPE THIS WILL FIX THE PROBLEM. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: REPORTER ADVISED HIS ACR UTILIZES DATALINK PDC FOR AIRWAYS CLRNCS WHERE AVAILABLE AND WAS TRUE IN THIS CASE. HE FURTHER STATED THAT ALTHOUGH HE IS UNAWARE OF A STRICT SOP FOR RECEIVING; REVIEWING AND PROGRAMMING THE FMS FROM THE PDC; A MORE OR LESS UNIVERSAL PRACTICE HAS BEEN ADOPTED WHEREBY THE CAPTAIN AND FIRST OFFICER REVIEW THE PDC PRINTOUT; MUTUALLY VERIFY THE CLEARED ROUTE AND ITS PROPER INSERTION IN THE FMS. HE BELIEVES THE ERROR IN THIS CASE WAS THE RESULT OF COMPLACENCY DUE TO MULTIPLE PAST DEPARTURES HERETOFORE ALWAYS EMPLOYING THE FILED ROUTE. THEY SIMPLY FAILED TO NOTE THE REVISION ON THE FLT IN QUESTION.
