B747-400 FLT CREW INADVERTENTLY TAXIED ONTO THE RWY IT WAS CLEARED TO CROSS.

Narrative

TAXI OUT FROM THE CARGO RAMP; VIA THE TAXIWAY A; G THEN CLEARED CROSS RWY 25L INITIALLY. DURING CROSS THE RWY 25L; ATC (TOWER) CLEARED CROSS RWY 25R THEN TAXIWAY B FULL LENGTH TO RWY 25R. BETWEEN RWY 25L AND RWY 25R THERE IS ONE TAXIWAY AC. UPON CROSSING THE AC TAXIWAY; I VERIFIED WITH FO; 'TAXIWAY B IS SECOND RIGHT (TURN) OR FIRST RIGHT.' FO SAID; 'FIRST RIGHT.' THEN I THOUGHT WE HAD PASSED THE RWY 25R (BUT ACTUALLY WAS THE TAXIWAY AC) SO I MADE RIGHT TURN TO TAXIWAY B (BUT ACTUALLY WAS TURNING TO RWY 25R) ALMOST AT THE SAME TIME; THE CREW (FO MAYBE FIRST) WAS AWARE THAT WE WERE TURNING TO RWY 25R (HAD ALREADY TURN 50-60 DEGREES; IT WAS LATE TURN BACK TO G TAXIWAY) SO WE INFORMED TOWER RIGHT AWAY AND REQUEST EXIT THE RWY BY FIRST LEFT TAXIWAY WF. THEN WE EXPEDITE EXIT RWY 25R VIA TAXIWAY WF THEN RIGHT TURN TO THE TAXIWAY B FULL LENGTH TO RWY 25R. MY CREW AND I HAD DISCUSSED WHAT COULD HELP TO AVOID THAT: 1) DURING CREW BRIEFING; I SHOULD EMPHASIZE THE TAXIWAY AC MORE; FOR EXAMPLE TAXI VIA TAXIWAY G WILL CROSS RWY 25L FIRST THEN CROSS THE TAXIWAY AC THEN CROSS THE RWY 25R THEN RIGHT TURN TO TAXIWAY B. 2) BEFORE/DURING CROSS SHOULD VERIFY TAXIWAY AND RUNWAY LIGHTS AND SIGNS MORE CAREFULLY. 3) FO SHOULD USE THE CHART AND COMPARE CAPT'S TAXI ROUTE. ALSO PROVIDE TIMELY TAXI ORIENTATION TO CAPT. 4) ALL CREWS SHOULD MAINTAIN ALERTNESS; WHEN IN DOUBT SPEAK IT OUT! I HAVE QUESTIONS FOR THE TAXIWAY AC WHICH IS BETWEEN THE RWY 25L AND RWY 25R: 1) HOW MANY SO CLOSE AND PARALLEL RUNWAYS HAVE THE TAXIWAY IN BETWEEN IN USA? 2) DURING DAYTIME MAYBE EASY TO IDENTIFY 25L THEN AC THEN 25R; BUT DURING NIGHTTIME IS IT THAT EASY TO IDENTIFY 25L; AC; 25R? 3) COULD THE LIGHTS AND SIGNS BE SURVEYED FOR PROPER LOCATION?
