Synopsis

MSP LCL CTLR EXPERIENCED OPERROR AT APPROX 1000 WHEN COORD BETWEEN LCL'S WAS CONFUSED AND TWR SUCCESSIVE DEPS CONFLICTED.

Narrative

CTLR #2 SAID THEY COORD A TURN INTO CTLR 1'S AIRSPACE. ACFT #1; OFF OF RWY 30R; WAS TURNED TO A HDG OF 260 DEGS. ACFT #2 DEPARTED RWY 30L AND WAS ALSO TURNED HDG 260 DEGS. I DID NOT RECALL THE COORD AND THE COORD WAS NOT RECORDED. THE PLT OF ACFT #2 SAW THE FIRST ACFT IMMEDIATELY AND STOPPED HIS CLB. CONTRIBUTING FACTORS: 1) ASR 11 RADAR WAS OTS. ACFT WERE NOT DISPLAYING ON THE RADAR FOR 1 1/2 - 2 MI. 2) AMASS WENT OFF LINE AT ABOUT THE SAME TIME. THIS CAUSED CONFUSION IN THE CAB. WE HAVE TO BRING OUT REMINDERS AND CHANGE OUR PROCS. THE PROB WAS FOUND WHEN WE OBSERVED THE A319 AT A LOWER THAN NORMAL ALT. ABOUT THE SAME TIME THE TARGETS STARTED TO SHOW UP ON THE RADAR. CORRECTIVE ACTIONS: 1) DO ALL COORD ON RECORDED LINES. NO CONFUSION ABOUT WHO SAID WHAT AND WHEN. 2) THE RADAR OUTAGE SHOULD NOT HAVE BEEN DURING DAYLIGHT HRS. THESE THINGS SHOULD BE DONE DURING MIDNIGHT OPS WHEN THERE IS LITTLE OR NO IMPACT. HUMAN FACTORS ARE ALWAYS AN ISSUE. DID SOMETHING HAPPEN? DO I REMEMBER THIS OR THAT? I DON'T KNOW HOW TO FIX THAT.
