Synopsis

LOSS OF NOSE STEERING ACCOUNT FLEX LINE BROKE. ACFT DAMAGED RESULT OF EXIT FROM TXWY. NO INJURIES.

Narrative

THE FLT WAS CLRD FOR A VISUAL APCH TO RWY 28R AT PIT; WITH THE F/O AT THE CTLS. INITIAL APCH CHKS INDICATED HYD SYS FUNCTIONING NORMALLY: BRAKE PRESSURE NORMAL; LNDG GEAR EXTENSION NORMAL. THE F/O LANDED THE AIRPLANE AND RELINQUISHED CTL TO THE CAPT ON ROLLOUT. THE CAPT EXITED THE RWY AT TXWY N5; A REVERSE HIGH SPD TXWY. WHILE TURNING FROM TXWY N5 ONTO TXWY N1 (PARALLEL TO RWY 28R); THE HYD SYS FAILED DUE TO A RUPTURED FLEX LINE TO THE NOSEWHEEL STEERING ACTUATOR. THE NOSEWHEEL WAS TURNED TO THE LEFT AND COULD NOT BE CENTERED; AND NORMAL WHEELBRAKES HAD FAILED. THE AIRPLANE CONTINUED TO TURN PAST THE CENTERLINE OF TXWY N1 AND BACK TOWARD RWY 28R. THE CAPT SELECTED EMER BRAKE PRESSURE AND STOPPED THE AIRPLANE ON THE GRASS BTWN RWY 28R AND TXWY N1. AS WE EXITED THE RWY; A WDB WAS CLRD FOR TKOF AND BY THIS TIME WAS ROLLING ON THE RWY AT A HIGH RATE OF SPD. THE LTT WAS NEAR ENOUGH TO THE RWY TO POSE A SERIOUS HAZARD; AND THE CAPT USED REVERSE PITCH TO BACK AWAY FROM THE RWY. THE LTT HAD TO RUN OVER A SECTION OF SNOW FENCE BTWN RWY 28R AND TXWY N1; AND AS THE AIRPLANE WAS BACKED UP MINOR DAMAGE WAS DONE TO THE AIRFRAME BY THE SNOW FENCE ANCHORS.
