FUEL LEAKED FROM FREIGHT IN CARGO OF FREIGHTER. DIVERTED TO CLOSEST SUITABLE ARPT.

Narrative

FLT XXX; AN ACR LGT B727-100 AIRPLANE; DEPARTED OAK AT XA51Z OCT/XA/94. THE AIRPLANE CLBED TO FL330 ENRTE TO MEM. AT ABOUT THE MINA VOR THE SO WENT TO THE BACK AND RETURNED RPTING THE STRONG ODOR OF FUEL IN THE CARGO CABIN. A RECHK OF THE CABIN REVEALED NO PHYSICAL EVIDENCE OF FUEL. THE SMOKE/FUMES CHKLIST WAS COMPLETED; THE COMPANY WAS NOTIFIED VIA COMPANY RADIO; AN EMER WAS DECLARED; AND THE AIRPLANE DIVERTED INTO LAS (A DISTANCE OF APPROX 130 NM). FUEL DUMP OF APPROX 3 1/2 MINS WAS COORDINATED WITH NELLIS RANGE CTL. A VISUAL APCH AND LNDG WAS ACCOMPLISHED ON RWY 19L AT LAS. CFR WAS STANDING BY AT THE RWY. UPON LNDG THE EMER WAS TERMINATED; AND 1 CFR TRUCK ACCOMPANIED THE AIRPLANE TO THE RAMP. A CARGO INSPECTION REVEALED JET FUEL LEAKING FROM A CONTAINER IN THE #2 POS. THE CONTAINER WAS REMOVED FROM THE AIRPLANE AND THE CONTENTS OF THAT CONTAINER WERE CHKED. A BOX; SHIPPED FROM MAINT; WAS FOUND TO BE LEAKING A CONSIDERABLE AMOUNT OF FUEL. THE BOX CONTAINED AN ENG FUEL CTL WHICH WAS BEING SENT TO MEM. THE AIRPLANE WAS RELOADED (SANS FUEL CTL) AND PROCEEDED TO MEM. AS A RESULT OF THE UNLOADING AND INSPECTION; IT WAS ALSO DISCOVERED THAT THE DANGEROUS GOODS FORM PART A WAS IMPROPERLY COMPLETED. THIS INCIDENT IS UNDER INVESTIGATION BY FLT SAFETY DEPARTMENT. ACTION TO BE TAKEN WILL ADDRESS: A) PROPER PREPARATION OF AVIATION PARTS AND EQUIP FOR SHIPPING; AND B) DANGEROUS GOODS SCREENING AND PROPERLY EXECUTED PAPERWORK PERTAINING THERETO.
