REPETITIVE; MULTIPLE ACFT EQUIP PROBS MALFUNCTIONS CAUSE FLT CANCELLATION ON AN A-320.
Narrative
THE ACFT WAS AT THE GATE HAVING BEEN RELEASED BY MAINT FROM A 'C' CHK. AS FO; I WAS FIRST TO ARRIVE AND FOUND ACFT WITH TOO MUCH FUEL FOR THE FLT TO TUS. NOTIFIED DISPATCH AND BEGAN PREFLT. FAA WAS ALSO ABOARD ACFT AS I PREFLTED. NO OTHER DISCREPANCIES WERE NOTED AT THAT TIME BY ME OR THE FAA. DURING FIRST ENG START; #1 ENG IGNITION 'B' FAILED. NOTIFIED DISPATCH AND RECEIVED AN 'UNMANNED STATION RELEASE.' WE CONTINUED ON TO TUS. AT LNDG; BOTH ENG THRUST REVERSERS FAILED TO DEPLOY; FOLLOWED BY #1 AND #2 FULL AUTH DIGITAL ENG CTL SYS FAIL LIGHTS. WE CONTINUED ON TO GATE AND CAPT CALLED MAINT CTL. WE RESET AFFECTED SYS IN ACCORDANCE WITH MAINT INSTRUCTIONS. SYS RESET NORMALLY AND WE CONTINUED BACK TO PHX. DURING FLT; #2 FUEL PUMP FAILED. UPON LNDG AT PHX; BOTH REVERSERS AND FULL AUTH DIGITAL ENG CTL SYS FAILED AGAIN. WE CONTACTED MAINT CTL AND MAINT WORKED ON ACFT FOR 2 1/2 - 3 HRS. THE ACFT WAS RELEASED BACK TO THE CREW. WE BOARDED; LOADED OUR PAX FOR FLT TO MKE. AT PUSHBACK CREW NOTICED THAT OVERWING EXITS WERE NOT ARMED. WE NOTIFIED MAINT CTL; CAME BACK TO GATE; AND FLT WAS CANCELED. SUPPLEMENTAL INFO FROM ACN 315294: ACFT PULLED TO GATE FOR FLT AFTER UNDERGOING 'C' CHK. PREFLT BY FO AND FAA INSPECTOR FOUND NO DEFECTS. HOWEVER; ACFT WAS OVERFUELED AND REQUIRED 3500 LBS DEFUEL. DURING FIRST ENG START 'B IGNITION FAULT' OCCURRED. CREW CONTACTED MAINT AND RECEIVED AN 'UNMANNED STATION AIRWORTHINESS RELEASE.' FLT DEPARTED FOR TUCSON; AZ. UPON LNDG IN TUCSON; BOTH #1 AND #2 ENG THRUST REVERSERS FAULTED; BOTH FADEC #1 AND #2 FAULTED. CAPT CONTACTED PHX MAINT CTL. CREW RESET SYS AS PER MAINT INSTRUCTIONS. FLT DEPARTED FOR PHX. DURING FLT L WING #2 FUEL PUMP FAILED. UPON LNDG IN PHX; BOTH REVERSERS AND FADECS FAILED AGAIN. CREW CONTACTED MAINT CTL. PHX MAINT WORKED ON ACFT FOR APPROX 2 1/2 - 3 HRS. AFTER COMPLETION OF MAINT; CREW REBOARDED ACFT FOR NEXT FLT LEG. ON PUSHBACK CREW FOUND OVERWING EMER EXITS WERE DISARMED. CREW CONTACTED MAINT CTL AGAIN. FLT WAS CANCELED.
