A CANADAIR CL65 IN DSCNT AT 11000 FT DECLARED AN EMER AND SHUT DOWN #2 ENG DUE TO LOW OIL PRESSURE WARNING AND INDICATION CAUSED BY THE LOSS OF OIL FROM A CUT SEAL ON THE ENG DRIVEN HYD PUMP DRIVE SHAFT.
Narrative
ACR FLT ZYYZ DEPARTED TORONTO (YYZ) AT XA15 ON JAN/XA/99 ENRTE TO CLT. ABOUT 40 MI FROM CLT WE STARTED GETTING INTERMITTENT LOW OIL PRESSURE INDICATIONS. I REVIEWED THE PROCS AND INFORMED ATC (ZTL) THAT WE NEEDED EXPEDITIOUS HANDLING TO CLT. ABOUT 25 MI FROM CLT THE LOW OIL PRESSURE READINGS BECAME CONSTANT (LESS THAN 25 PSI) AND A PRECAUTIONARY ENG SHUTDOWN ON THE #2 ENG WAS COMPLETED. I DECLARED AN EMER AND THE FLT LANDED WITHOUT INCIDENT. CAUSE: THE ACFT (XYZ) HAD HAD A HYD PUMP ON THE #2 ENG REPLACED IN ZZZ ABOUT 5 FLT HRS PRIOR TO THIS INCIDENT. THE PUMP WAS INSTALLED POORLY. A SEAL ON THE PUMP SHAFT HAD BEEN NICKED CAUSING THE ENG TO LEAK OIL INTERNALLY AND VENT IT OVERBOARD. THE ACFT USES GENERAL ELECTRIC CF34-3B1 ENGS AND NONE OF OUR A&P'S HAVE HAD ANY TRAINING ON THIS ENG. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THIS R ENG DRIVEN HYD PUMP SHAFT WAS INSTALLED 5 HRS FLT TIME PRIOR TO THIS FAILURE. THE RPTR SAID THERE MUST BE SOME INSTALLATION PROB WHEN CHANGING THIS SEAL BECAUSE ANOTHER FAILURE HE KNEW OF WAS A DAMAGED SEAL ALSO. THE RPTR STATED IT WAS NOT A MASSIVE OIL LEAK BUT JUST A STEADY DRIPPING.
