Synopsis

A B727-200 WAS DISPATCHED WITH THE #1 ENG OIL CAP NOT SECURED INCURRING A LOSS OF OIL.

Narrative

FLT ABC (ACR) LEFT YYY TO XXX ON JAN/XY/99. WHILE ENRTE TO XXX FE ON B727 NOTICED #1 ENG OIL QTY GO FROM 3 GALS TO 1 GAL. CAPT PULLED #1 ENG THROTTLE TO IDLE AND CONTINUED TO XXX. LOGBOOK WRITE UP WAS '#1 ENG OIL QTY DECREASED FROM 3 GALS TO 1 GAL AND STABILIZED. OIL TEMP AND PRESSURE REMAINED NORMAL.' CORRECTIVE ACTION WAS 'FOUND OIL CAP OFF OIL TANK; PUT IN 12 QTS OF OIL.' THE B727 SHIP #XYZ HAD A SCHEDULED SVC CHK THE NIGHT BEFORE ON JAN/XZ/99. AT XA03 THE B727 ARRIVED AT YYY. MYSELF AND 2 OTHER MAINT TECHNICIANS WERE ASSIGNED TO WORK ON THE ACFT. I DID THE ENG ITEMS FOR THE #1 ENG WHICH WERE REPLACE ENG OIL FILTER; HYD FILTER; SVC CSD WET SPLINE; SVC ENG OIL TO FULL; CHK CHIP DETECTOR. AFTER ALL THOSE ABOVE ITEMS HAD BEEN ACCOMPLISHED I HAD THE OTHER TECHNICIAN RUN #1 ENG FOR LEAK CHK. NO LEAKS WERE FOUND. I CLOSED THE ENG COWLING AND DECIDED I SHOULD CHK THE OIL QTY ONE MORE TIME. I OPENED THE OIL ACCESS DOOR REMOVED OIL CAP AND PUT IN 2 QTS OF OIL. I REMEMBER PUTTING THE OIL TANK CAP BACK ON. WHEN THE OTHER TECHNICIAN YELLED SOMETHING TO ME FROM THE GND ABOUT HE WAS GOING TO START ON THE DOUBLE BRAKE CHANGE AND THAT HE WAS ONLY ABLE TO FIND 1 BRAKE. I TURNED MY ATTN TO HIM AND CALLED THE LEAD TECHNICIAN ON THE RADIO TO INFORM HIM OF THE SIT. AFTER THAT I LOOKED BACK AT THE ENG AND CLOSED THE OIL SVC DOOR. I DON'T RECALL IF I SECURED THE OIL CAP LOCK LEVER. I THEN LOWERED THE LIFT TRUCK DOWN AND WENT TO HELP THE OTHER TECHNICIAN WITH THE DOUBLE BRAKE CHANGE. WITH SO MANY THINGS GOING ON AT ONE TIME IT'S NOT HARD TO GET DISTRACTED.
