Synopsis

A CL65 FLC ENTERS THE HOLDING PATTERN NEAR SLT IN THE WRONG DIRECTION WITH THE INCORRECT RADIAL SET IN THEIR FMS.

Narrative

ENRTE FROM CVG TO EWR; WE WERE CLRED TO HOLD AS PUBLISHED OVER SLT. BECAUSE I WAS THE PF; THE CAPT WAS ASSUMING THE RESPONSIBILITY OF PROGRAMMING THE FMS FOR THE HOLD. INSTEAD OF SETTING THE FMS UP FOR A HOLD 107 DEG RADIAL; IT WAS INADVERTENTLY SET UP ON THE 287 DEG RADIAL. ATC CAUGHT THE ERROR BEFORE WE DID; NOTICING THAT WE HAD GONE PAST THE VOR AND HAD TURNED ABOUT 10 DEGS TO THE R AS THE PLANE WAS ATTEMPTING A TEARDROP ENTRY. IT WAS THE R TURN THAT CAUGHT MY ATTN. THIS COULD HAVE BEEN PREVENTED IF I HAD BEEN MORE INVOLVED IN BRIEFING AND SETTING UP THE HOLD. INSTEAD; I HAD JUST ASSUMED CTL OF THE RADIOS WHILE HE PROGRAMMED THE FMS. LIKEWISE; HE COULD HAVE SOUGHT MY VERIFICATION OF THE FMS HOLD PAGE PRIOR TO EXECUTING. UNFORTUNATELY; WE WERE BOTH A LITTLE TOO PRESUMPTIVE; AND ATC CAUGHT US -- WHAT IF THEY HAD NOT? SUPPLEMENTAL INFO FROM ACN 436608: JUST ANOTHER INCIDENT DUE TO A DAY FILLED WITH CANCELLATIONS; WX. IT; IN ITSELF; WAS DUE TO THE FACT WE DID NOT VERIFY WITH EACH OTHER; A CLASSIC CASE OF COMPLACENCY.
