B757-200. ACFT ROLLED INTO JETWAY AT GATE AREA AFTER THE PARKING BRAKE RELEASED AND THE GND CREW DID NOT CHOCK THE WHEELS.
Narrative
AFTER ARR AT THE GATE THE CAPT PARKED THE BRAKES AND THE FO SHUT DOWN THE L ENGINE. THE RIGHT ENGINE WAS STILL RUNNING. THE GND CREW SIGNALMAN GAVE THE 'CHOCKS IN' SIGNAL. THE FO CONTINUED WITH SECURE COCKPIT DUTIES AND THE CAPT BEGAN COMPLETING THE LOGBOOK ENTRIES WHILE WAITING FOR EXTERNAL PWR TO BE PLUGGED IN. THE PARKING BRAKE LATCH APPARENTLY DID NOT GET FULLY LATCHED AND RELEASED WITHOUT EITHER PLT BEING AWARE. THE ACFT SLOWLY STARTED TO ROLL AND THE L ENGINE CONTACTED THE JETWAY CAUSING DAMAGE TO THE ENGINE COWL. EVEN THOUGH THE GND CREW HAD GIVEN THE 'CHOCKS IN' SIGNAL; THEY HADN'T ACTUALLY PUT THE CHOCKS IN. WHEN THEY SAW THE AIRPLANE START MOVING; THE SIGNAL MAN WENT BACK TO THE FRONT OF THE AIRPLANE; PICKED UP HIS WANDS; AND TRIED TO SIGNAL THE CAPT TO STOP THE AIRPLANE. THE FIRST CORRECTIVE ACTION WOULD BE TO TRIPLE CHK; NOT JUST DOUBLE CHK PARKING BRAKE ENGAGEMENT. THE SECOND WOULD BE TO RE-EMPHASIZE WHAT WE ALREADY KNOW - DON'T ALLOW YOURSELF TO DE DISTRACTED BY UNIMPORTANT DUTIES. THIRD WOULD BE TO ENCOURAGE GND CREWS TO BE MORE AGGRESSIVE IN TAKING ACTION WHEN THEY SEE SOMETHING OBVIOUSLY WRONG.
