A B757-200 WAS DISPATCHED IN NON COMPLIANCE WITH THE FORWARD ENTRY DOOR ESCAPE SLIDE AND DOOR ASSIST BOTTLE NOT SVCABLE.
Narrative
AT DEP TIME; I HEARD 2 QUICK KNOCKS ON THE FORWARD ENTRY DOOR AND AN IMMEDIATE OPENING AND SUBSEQUENT CRASH OF THE EMER EXIT SLIDE HIT THE JETBRIDGE FLOOR. THEN A GND PERSON HANDED ME A HAZMAT SLIP; INFECTIOUS TO HUMANS; FORM AND QUICKLY LEFT. WE IMMEDIATELY CALLED MAINT AND EXPLAINED ABOUT THE SLIDE PACKAGE. THE #1 FLT ATTENDANT TOLD ME SHE DID NOT HAVE AN OPPORTUNITY TO OPEN THE DOOR SINCE IT WAS OPENED BY SOMEONE OUTSIDE THE ACFT. I AGREED; SINCE I NOTICED HOW QUICKLY IT WAS OPENED AFTER THE 2 KNOCKS. I FOUND OUT THAT THE DOOR WAS ALREADY ARMED AND UPON LOOKING OVER THE SIT; NOTICED THAT THE DOOR ARMED HANDLE WAS CURIOUSLY STILL IN THE ARMED POS. WITH THE DOOR OPEN AND THE SLIDE PACKAGE LYING ON THE JETBRIDGE FLOOR. I POINTED THIS OUT TO MAINT AS I ASKED THEM IF THEY WANTED ME TO ENTER IT INTO THE LOGBOOK. THEY SAID NO; THAT THEY WOULD TAKE CARE OF IT. I DID ASK THEM A SECOND TIME IF THEY WANTED ME TO ENTER IT INTO THE LOG AND THEY SAID NO A SECOND TIME. IT TOOK MAINT ABOUT 35 MINS OR SO TO GET THE SLIDE READY TO GO; OR SO I THOUGHT. INFLT; THE #1 FLT ATTENDANT SAID SHE FELT THAT THE GIRT BAR WAS HANGING LOWER THAN IT SHOULD BE; SO THE FO AND I EACH TOOK A LOOK AT IT ON BATHROOM BREAKS ENRTE. I OBSERVED THAT THE DOOR WAS ARMED AND REMEMBERED THAT IT WAS STILL ARMED WITH THE DOOR OPENED WITH THE SLIDE PACKAGE ON THE JETBRIDGE FLOOR AT ZZZ. I SENT A MESSAGE VIA ACARS TO MAINT AND ASKED THAT A MAINT REPRESENTATIVE MEET THE ACFT AT THE JETBRIDGE BEFORE THE DOOR WAS OPENED. I FOLLOWED UP WITH A RADIO CALL TO MAINT ON THE WAY IN. I ALSO COORD WITH THE FLT ATTENDANT THAT SHE WAS TO KEEP ALL OF THE PAX AWAY FROM THE DOOR UNTIL MAINT SAID IT WAS SAFE. UPON ARR; THE ACFT WAS MET BY SEVERAL MAINT PERSONNEL; AND 2 SHOWED ME THAT THE EMER DOOR BOTTLE WAS DISCHARGED AND PROBABLY WAS DISCHARGED AT ZZZ. I DO NOT RECALL EVEN HEARING IN TRAINING THAT THERE WAS A BOTTLE TO ASSIST WITH OPENING THE DOOR IN AN EMER. SLIDE PROBABLY DIDN'T GET FIXED PROPERLY BACK AT ZZZ. WE HAD FLOWN ALL THE WAY TO ZZZ1 WITH A SLIDE THAT WAS NOT ARMED. I TOLD HIM THAT ENRTE; I OBSERVED THAT THE DOOR WAS IN THE ARMED POS; THAT I DID NOT KNOW ABOUT THE EMER BOTTLE OR THAT IT WAS DISCHARGED. I FEEL THAT MAINTAIN ZZZ SHOULD HAVE CHKED THAT BOTTLE IN ZZZ BEFORE RELEASING ACFT FOR FLT. THE MAINT SUPVR ALSO TOLD ME MY OBSERVATION AT ZZZ THAT THE DOOR ARMED HANDLE STILL IN THE ARMED POS WITH THE DOOR OPEN WAS INFO TO HIM; THAT THE DOOR HAD BEEN OPENED FROM THE INSIDE AT ZZZ. MY FLT ATTENDANT SAID NO ONE INSIDE THE ACFT OPENED THE DOOR. THE ONLY THING I WISH I HAD DONE DIFFERENTLY WOULD HAVE BEEN TO ENTER THE PROB INTO THE LOGBOOK AS I NORMALLY WOULD HAVE. MAYBE I RUSHED TO ACCOMMODATE THE PAX CONNECTIONS.
