B757-232 EXPERIENCES DISINTEGRATION OF R CSD WHICH RESULTS IN LOSS OF ENG OIL; ENG SHUT DOWN AND DIVERSION TO NEAREST SUITABLE ARPT.
Narrative
THE COCKPIT LIGHTS FLASHED AND THE INSTS WENT BLANK FOR APPROX 1 SECOND. EICAS AND ELECTRICAL PANEL INDICATIONS ALERTED US THAT A R GENERATOR DRIVE FAILURE HAD OCCURRED. APPROPRIATE ABNORMAL PROC WAS ACCOMPLISHED TO INCLUDE DISCONNECT OF R GENERATOR DRIVE AND STARTING APU. FLT WAS CONTINUED AS PLANNED. 5 MINS LATER; THE R ENG OIL QUANTITY WAS OBSERVED AS ZERO QUARTS. PRESSURE REMAINED NORMAL FOR APPROX 2 MINS THEN STARTED TO FLUCTUATE AND STEADILY DECREASE. IT WAS DETERMINED THAT ENG SHUTDOWN WAS INEVITABLE AND IMMINENT. A NORMAL DSCNT TO DRIFT-DOWN ALT WAS INITIATED. FLT CTL WAS CONTACTED TO ADVISE OF ENG SHUTDOWN AND INTENT TO DIVERT. FLT CTL AND CREW AGREED THAT ZZZ WAS PREFERRED. ENG WAS SHUT DOWN USING ABNORMAL PROC GUIDANCE. FLT ATTENDANTS AND PAX WERE NOTIFIED OF THE CIRCUMSTANCES. EMER WAS DECLARED WITH CTR AND DIVERT TO ZZZ WAS COMMENCED. DSCNT AND LNDG WERE NORMAL AND UNEVENTFUL. ACFT WAS TAXIED CLR OF RWY 25R AND INSPECTED BY ARRF TO DETERMINE SUITABILITY FOR CONTINUED TAXI. TAXIED TO GATE AND DEPLANED PAX. ENG OIL FAILED APPARENTLY DUE TO CATASTROPHIC FAILURE OF R ENG CSD SEIZING AND THROWING CHUNKS THROUGH THE CSD CASE. THE PROB WAS DISCOVERED THROUGH EICAS ALERTING; UNUSUAL COCKPIT INDICATIONS AND NORMAL INST MONITORING. CORRECTIVE ACTIONS WERE IN ACCORDANCE WITH ABNORMAL PROCS CHKLISTS AND NORMAL OPERATING CHKLISTS. HUMAN FACTORS CONSIDERATIONS WERE TEXTBOOK BY TRAINING. CREW DIVIDED RESPONSE: CAPT FLEW; NAVIGATED AND COMMUNICATED WITH ATC; FO (PNF) HANDLED CHKLIST AND COMS WITH FLT ATTENDANTS AND COMPANY UNTIL SITUATION WAS STABILIZED. ALL APPROPRIATE ACTIONS WERE COMPLETED AND REVIEWED.
