Synopsis

A B757-200 ACFT #1 ENGINE NOSE INLET COWL WAS REPLACED DUE TO DAMAGE. PREVIOUSLY DISCONNECTED ENG ANTI-ICE VALVE CANON PLUG NOT RECONNECTED.

Narrative

DURING REVIEW OF OUR FLT PAPERS; NO ITEMS WERE LISTED IN THE DISPATCHER COMMENT SECTION OR IN THE 'DISPATCH ITEMS' OR 'MEL DEV LIST ITEMS' SECTIONS. DURING PREFLT; A DEFERRED STICKER WAS NOTICED ON THE OVERHEAD INST PANEL NEAR THE L ENG ANTI-ICE VALVE SWITCH. AFTER CONSULTING THE MAINT RELEASE DATED AUG/XA/07; NO DEFERRED ITEMS WERE LISTED. UPON FURTHER INSPECTION; IT WAS NOTED THAT DEFERRAL X REFED LOG ITEM NOSE INLET COWL REPLACEMENT DUE TO DAMAGE AND THE DEACTIVATION OF THE ENG ANTI-ICE VALVE. THIS ITEM WAS CLRED UNDER THE ACTION CODE 'RPL.' IMMEDIATELY AFTER ENG START; THE L ENG ANTI-ICE VALVE SWITCH LIGHT WAS ILLUMINATED INDICATING THAT THE VALVE WAS IN TRANSIT/DISAGREEMENT WITH THE SWITCH POS. CYCLING THE SWITCH WAS NO HELP. A LOG ITEM WAS ENTERED INTO THE LOGBOOK. AN ACARS MESSAGE WAS SENT TO DISPATCH NOTIFYING THE DISPATCHER OF THE MAINT ACTIVITY AND THE NEED TO NOTIFY MAINT CTL OF THE MAINT ACTIVITY. RADIO CONTACT WAS MADE WITH LCL MAINT WHO SUBSEQUENTLY ASSIGNED MAINT PERSONNEL TO OPEN THE FAN COWL FOR TROUBLESHOOTING. THE ENG WAS SECURED TO ALLOW MAINT ACCESS. THE MECH RPTED THAT THE VALVE'S CANNON PLUG WAS FOUND DISCONNECTED AND THE VALVE WAS SAFETY-WIRED CLOSED. THE SAFETY WIRE WAS REMOVED AND THE CANNON PLUG RECONNECTED. THE VALVE OPERATED NORMALLY. THE OPEN LOG ITEM WAS SIGNED OFF AND A NEW MAINT RELEASE ISSUED. THE ANTI-ICE VALVE OPERATED NORMALLY FOR THE REMAINDER OF THE FLT.
