Synopsis

A CRJ CAPT RPTS IMPROPER DE-ICE PROCS THAT LEFT HIS ACFT WITH ICE CONTAMINATION AFTER IT WAS RPTED CONTAMINANT FREE.

Narrative

CRJ PARKED AT GATE COVERED IN 1/4-1/2 IN ICE. STATION RPTED MAJOR ICE DEPOSITION CEASED AT XA30. ARRIVED AT XC00 TO DISCOVER WALKWAYS COVERED IN ICE; NOT SALTED; COVERED IN ICE; NOT DE-ICED. STATION RPTED FOUR DEPS IN THE XC30 TO XD30 TIMEFRAME. THOUGH 3 HRS HAD PASSED; THE RAMP AND FOUR PLANES WERE NOT DE-ICED UNTIL CREWS MADE A REQUEST. ACFT WERE DE-ICED WITH TYPE 1 AND ANTI-ICED WITH TYPE 4. DE-ICE TRUCK DEPARTED AND RPT WAS MADE THAT ACFT WAS FREE OF CONTAMINANTS. CREW OBSERVED 1/4 INCH OF ICE STILL ON WINDSHIELD WIPERS. SUSPICIOUS OF FURTHER ICE; THE CAPT MADE VISUAL AND TACTILE INSPECTION AND OBSERVED ICE ON WIPERS; NOSE CONE; TOP OF FUSELAGE; TOP OF WING ROOTS; AND TAIL. THE TRUCK WAS CALLED BACK AND PROCS WERE COMPLETED TO ENSURE ACFT WAS INDEED CONTAMINANT FREE! CONCERN IS THAT THE ACFT SHOULD NOT HAVE BEEN RPTED AS CONTAMINANT FREE THE FIRST TIME WHEN IT WASN'T. RAMP ALSO HAD UNCERTAINTY AS TO WHERE TO CONDUCT DE-ICING OF THE CRJ. FIRST SUGGESTION WAS TO PUSH ONTO THE RAMP WHICH WOULD HAVE TIED UP THE RAMP FOR 45 MIN TO AN HOUR. EVENTUALLY DECISION WAS MADE TO DE-ICE THE ACFT IN PLACE. THE STATION ELECTED NOT TO PARK THE CRJ AT A JETWAY BUT FAILED TO SALT THE WALKWAYS PRIOR TO CREW ARR. THE FLT ATTENDANT SLIPPED PULLING A MUSCLE IN BACK. WHEN REQUESTED BY THE CREW; THE WALKWAY WAS SUBSEQUENTLY SALTED; BUT BARELY ADEQUATELY. MY FO AND I OBSERVED RAMP PERSONNEL FALL TO THE GND ON THREE OCCASIONS DUE TO THE ICY RAMP.
