What happened
On May 27, 2018, at 11:17 UTC, an XtremeAir XA42, registration D-EIXA, was performing an aerobatic flight near the Ponte de Sor Airfield (LPSO) in Portugal. A few minutes after takeoff, the aircraft suffered a loss of engine power. While attempting to return to the airfield, the pilot declared an emergency and initiated procedures for a forced landing. The aircraft subsequently performed an off-field landing on unprepared terrain, which resulted in the aircraft flipping over. The pilot sustained minor injuries, while the passenger remained uninjured. The aircraft suffered substantial damage.
The investigation
The GPIAAF investigation examined the aircraft's maintenance records, airworthiness documentation, and the engine and propeller condition. The investigation confirmed that the aircraft's documentation was valid and that it had been maintained according to approved procedures. The mass and center of gravity were found to be within established limits. Examination of the wreckage revealed that propeller blade damage was consistent with an engine power loss at the time of impact. No mechanical defects or structural failures were identified as contributing to the accident.
Findings
- The engine stopped due to fuel starvation.
- The pilot failed to perform physical fuel quantity checks before and during the flight.
- The operational environment of the airshow/event acted as a distraction, leading to poor fuel management.
- This was the only flight among ten performed at the airfield that involved different loading parameters due to the presence of a passenger.
- There was no evidence of any pilot error regarding flight maneuvers or incorrect handling of the aircraft prior to the power loss.
