What happened
On February 16, 2022, at approximately 17:40 local time, an MD Helicopters 600N was performing a non-scheduled passenger transport flight in the Cochamó commune, Los Lagos Region. While attempting to depart from a clearing on Cerro El Morro, the aircraft began an uncontrolled turn to the right. Recognizing the deviation, the pilot executed an emergency landing at the same location from which the flight had originated.
Despite the sudden maneuver, all five occupants (the pilot and four passengers) were able to exit the aircraft on their own. There were no injuries reported following the event. The aircraft sustained significant damage to the fuselage and the main rotor blades.
The investigation
DGAC Chile investigators examined the aircraft, the site, and the operational conditions. The investigation confirmed that the aircraft's maintenance records were up to date, with the last periodic inspection completed in December 2021. The aircraft's weight and balance were within certified limits at the time of the incident, with a takeoff weight of 3,831.5 pounds, below the maximum allowable 3,850 pounds.
At the time of the occurrence, meteorological conditions in the area were favorable, characterized by clear skies and high visibility. The investigation also noted that the aircraft's communication equipment was functional and that all safety restraints, including seatbelts and harnesses, were in good condition and properly utilized by the occupants.
Findings
- The aircraft experienced a rightward deviation immediately following takeoff.
- The main rotor blades and the fuselage sustained substantial structural damage.
- The aircraft was found resting at a 15-degree rearward tilt, with the rear skids embedded in the ground.
