Accident Details
On June 21, 2008, at approximately 1800 central daylight time, a Cessna R172K (registration N1439V) was substantially damaged during an aborted takeoff at David J. Perry Airport (1K4) in Goldsby, Oklahoma. The private pilot was operating the personal flight under Title 14 Code of Federal Regulations Part 91 without a flight plan. Visual meteorological conditions prevailed at the time. The cross-country flight was originating from Goldsby and was en route to Joplin Regional Airport (JLN) in Joplin, Missouri. The pilot and two passengers were not injured.
According to an FAA inspector, the pilot reported that he did not believe the airplane was accelerating as expected and therefore decided to abort the takeoff. The airplane continued past the end of the runway for approximately 500 feet before nosing over, which resulted in crushing the vertical stabilizer.
Pilot Information
Following the accident, numerous attempts were made to contact the pilot to request submission of NTSB Form 6120.1-2, but the pilot did not respond. The FAA's Oklahoma City Flight Standards District Office noted that the pilot twice failed 14 CFR Part 44.709 reexamination checks—on September 16, 2008, and again on November 12, 2008. The pilot also failed to appear for scheduled reexaminations, leading to efforts to revoke his pilot certificate pending compliance.
