Accident Details

On January 7, 2009, at approximately 1100 central standard time, an Inman Kolb Mark III airplane, registration N313JD, was substantially damaged during a forced landing at a private airstrip near Ozark, Arkansas. The pilot, who held a sport pilot certificate, was seriously injured. Visual meteorological conditions prevailed at the time, and the personal flight was conducted under 14 CFR Part 91 without a flight plan.

Pilot Actions

According to a statement from the pilot, he added 5 gallons of 91-octane automobile fuel to the fuel tank prior to the flight. A sump check of the fuel did not reveal any contaminants. He completed a 10-minute local flight and landed, then decided to take off again. During the second takeoff, the pilot rotated the airplane at 65 miles per hour. When the airplane reached about 20 feet above ground level, the engine stopped producing power. The pilot was unable to pitch the nose down before the airplane stalled. The airplane descended and experienced a hard landing. During the landing, the airplane departed the runway and collided with a berm. Substantial damage was sustained to the airplane's left aileron.

Investigation

An on-scene examination established control continuity and throttle continuity. Two fuel samples taken from the airplane's fuel sump drain revealed the presence of white particles. The source of the fuel contaminants could not be determined. No other anomalies were noted.
