A helicopter lost engine power during a low-level spray pass due to fuel exhaustion after the pilot miscalculated fuel quantity during refueling. The aircraft auto-rotated and came to rest on its side.

Incident Details

The pilot reported that while conducting an aerial application flight, he lined up for a downwind spray pass at an approximate height of three feet above the crop. During the pass, the engine sputtered and then lost power. The pilot entered an autorotation and attempted to slow the aircraft from 50 MPH before the helicopter impacted terrain and came to rest on its side.

Investigation Findings

An FAA Inspector's report noted that the helicopter had been refueled for the second time during the job about one hour prior to the accident. The refueling did not bring the helicopter to its proper fuel quantity, causing the pilot to miscalculate the fuel on board. Post-impact inspection revealed an estimated two gallons or less of fuel in the tank, with no ruptures found in the fuel tank.
