Accident
On May 14, 2013, at approximately 1050 central daylight time, a Piper PA-36-285 airplane (registration N57719) conducted a forced landing in an open field near Galena, Kansas. The commercial pilot sustained minor injuries. The airplane, registered to and operated by Four States Sky Ag Inc., was being used for an agricultural application flight under 14 Code of Federal Regulations Part 137. Visual meteorological conditions prevailed, and no flight plan was filed. The flight had departed Swalley Airpark (78KS), Baxter Springs, Kansas, about 1045.
Pilot Statement
According to a statement provided by the pilot to responding Federal Aviation Administration (FAA) inspectors, after departure, he had just started to level off when he noticed a decrease in oil pressure. Before he could set up for a forced landing, the engine experienced a total loss of power. The airplane subsequently collided with terrain.
Examination
FAA inspectors examined the engine and found that the number 2 cylinder piston had failed. A large amount of aluminum particles was observed in the oil and oil filter. Additionally, each spark plug was equipped with two spark plug washers.
Maintenance Records
Review of maintenance records revealed that the recommended oil change time had been exceeded. The mechanic who performed the last annual inspection stated that he had not installed the extra spark plug washers and was not aware of their presence on the engine. It could not be determined who installed the additional spark plug washers.
