Accident Overview
On February 24, 2014, at approximately 1330 central standard time, a Robinson R44II helicopter, registration N360AH, rolled over while hovering in Presidio, Texas. The aircraft was operated by a private pilot under 14 Code of Federal Regulations Part 91 as a personal flight. Visual meteorological conditions prevailed, and no flight plan was filed. The local flight originated from Presidio.
Sequence of Events
The pilot reported that he was surveying ranch land and had been hovering at an altitude of 12 to 15 feet above the ground. While transitioning to forward flight, he was unable to maintain sufficient power and altitude. The low rotor rpm warning horn sounded. In response, the pilot decreased the collective pitch in an attempt to "fly out of the situation," but the helicopter was too close to the ground. The helicopter subsequently lost tail rotor effectiveness, completing one revolution before contacting the ground and rolling over.
Injuries and Damage
The pilot and one passenger were not injured. A second passenger sustained serious injuries. The helicopter was substantially damaged.
