A charter flight operating a British Aerospace HS 125-700A crashed into a residential building during an approach to Akron Fulton International Airport.

What happened

On November 10, 2015, at approximately 1453 EST, an Execuflight charter flight was performing a nonprecision localizer approach to runway 25 at Akron Fulton International Airport (AKR). The aircraft, a British Aerospace HS 125-700A bearing registration N237WR, departed controlled flight and struck a four-unit apartment building in Akron, Ohio.

The flight originated from Dayton-Wright Brothers Airport (MGY) at approximately 1413 EST. At the time of the accident, the aircraft was operating under instrument flight rules (IFR) in instrument meteorological conditions (IMC) with a visibility of 1.5 statute miles and winds from 240 degrees at 08 knots.

The crash resulted in 9 fatalities, including the captain, the first officer, and seven passengers. There were no injuries to individuals on the ground. The aircraft was destroyed by the impact and a subsequent postcrash fire.

Probable cause

The accident was caused by the flight crew's mismanagement of the approach and several deviations from company standard operating procedures, resulting in an unstabilized approach, descent below the minimum descent altitude without visual contact with the runway, and an aerodynamic stall. Contributing factors included the operator's inadequate oversight, training, and safety program, as well as insufficient FAA oversight of the company's operations.
