Parachute malfunction during jump at Radawiec airfield

Casualties unknown • Radawiec EPLR, PL

A skydiver experienced a significant malfunction of their main parachute during a jump near Radawiec, necessitating the deployment of a reserve canopy.

What happened

On August 16, 2015, a skydiver with 252 previous jumps was performing a descent from an altitude of 3,000 m at the Radawiec (EPLR) airfield. During the freefall phase, the individual was filming the descent with a camera. At approximately 1,000 m, the skydiver initiated the deployment of the main parachute, a Spectre 190.

According to the individual's account, the deployment of the main canopy was performed in a stable and correct body position. However, during the inflation process, the suspension lines began to undergo an intensifying twist. Due to this malfunction, the skydiver executed emergency procedures by cutting away the main canopy and deploying the reserve parachute. The reserve canopy fully inflated at an altitude of approximately 700 m, allowing for a safe landing at the airfield.

The investigation

The investigation was conducted by the user/operator of the equipment. The inquiry focused on the sequence of the deployment and the mechanical behavior of the lines during the inflation of the Spectre 190 canopy.

Probable cause

The specific cause of the line twisting during the deployment of the main canopy was not established.
