What happened
On September 5, 2015, during a training session at the Radawiec (EPLR) airfield, a student skydiver performing their 13th jump experienced an incident during the deployment process. While executing a jump for the SL-1 task, the student's legs became entangled in the parachute lines during the opening sequence. After an unsuccessful attempt to free their legs from the lines, the student managed to detach the main canopy straps and deploy the reserve parachute. The student landed outside the airfield perimeter; however, the landing resulted in no injuries.
The investigation
The investigation, conducted by the training organizer, focused on the sequence of the deployment and the physical positioning of the student during the exit from the aircraft. The inquiry established that the entanglement was a direct result of the student's body position during the separation from the aircraft.
Findings
- The primary cause of the incident was the student adopting an incorrect body posture during the separation from the aircraft.
Safety action
Following the incident, the training organizer reviewed the event with all student skydivers before proceeding with subsequent jumps.
