What happened
On May 31, 2018, a pilot and a passenger departed from an airfield in Bobrowniki for a recreational sightseeing flight, intended as a birthday surprise for the passenger. During the flight, the pilot performed several high-bank turns at very low altitudes, occasionally flying below the tree line and directly over vehicles traveling on the DK70 highway. The aircraft's flight manual limits bank angles to 60 degrees, yet the pilot performed maneuvers exceeding these parameters.
Upon returning to the airfield, the pilot attempted an aerobatic maneuver similar to a roll. As the pilot transitioned from the maneuver into a climb, the structural integrity of the aircraft failed, causing the right wing to detach. The departing wing struck the cockpit canopy, scattering debris and cabin contents across a wide area. Following the structural failure, the aircraft entered an uncontrolled descent and struck the ground at a steep angle near the airfield at approximately 09:55 LMT. The impact resulted in two fatalities, as both the pilot and the passenger died at the scene. The QUALT 200 L, registration SP-SYGT, was completely destroyed.
The investigation
The PKBWL investigation examined the aircraft's structural history and the pilot's flight profile. Investigators found that the pilot had performed various modifications to the aircraft, including the use of adhesive tape to secure wiring for lights intended for night flight, despite the aircraft not being certified for night operations. The investigation also noted that the aircraft had undergone wing repairs and inspections in 2016. While the combined weight of the pilot and passenger slightly exceeded the maximum allowable crew weight, the commission determined that the primary cause was structural failure due to unauthorized maneuvers.
