A helicopter crashed into high-voltage power lines near Loxahatchee, Florida, resulting in two fatalities.

What happened

On March 18, 2022, at 14:32 EDT, an MD Helicopters 369HM, registration N650AG, crashed into high-voltage power lines near Loxahatchee, Florida. The flight, operated under 14 CFR Part 91 as a personal flight, departed Antiquers Aerodrome (FD08) in Delray Beach at approximately 13:59.

ADS-B data indicated the aircraft flew north over swampy state land at altitudes ranging from 25 to 75 feet above ground level and speeds between 83 and 100 knots. The power lines involved in the collision were positioned at 80 feet above ground level. The impact resulted in 2 fatal injuries, claiming the lives of both the commercial pilot and the pilot-rated passenger.

The investigation

Investigators located the wreckage in a swampy field, noting that power lines were found on the ground near the site. The wreckage path extended approximately 250 feet on a 270° magnetic course. The aircraft came to rest on its right side, with the skids separated from the airframe and all four main rotor blades fractured from the hub. One rotor blade was not recovered from the swamp.

Evidence of impact with the power lines was found on the skid brackets and the landing light housing. While parts from the engine, cowling, and tail section were scattered along the wreckage path, the instrument panel remained intact. An examination of the engine and airframe showed no evidence of mechanical malfunctions prior to the impact. Additionally, toxicology results for the pilot showed no drugs or alcohol.

At the time of the accident, the pilot held commercial ratings for airplanes but did not possess a rotorcraft-helicopter rating; his records showed only an endorsement for solo student flight in helicopters that prohibited carrying passengers. The passenger held airplane ratings but had no helicopter experience.

Probable cause

The pilot's failure to maintain clearance from power lines while maneuvering at a low altitude.
