Accident Details

On July 26, 2023, at about 1730 Pacific daylight time, a Maule M-5-235C, registration N6201M, was substantially damaged when it experienced a loss of engine power during the final approach to Orcas Island Airport (ORS), Eastsound, Washington. The pilot and a passenger sustained minor injuries. The flight was conducted under Title 14 Code of Federal Regulations Part 91 as a personal flight.

Flight History

The pilot reported departing Columbia Gorge Regional/The Dalles Municipal Airport (DLS), The Dalles, Oregon, with 45 gallons of fuel on board. After arriving at ORS, the pilot estimated approximately 15 gallons of fuel remained, with the left tank containing substantially more fuel than the right. The pilot then conducted two 15-minute sightseeing flights around the island with different friends. On both flights, the fuel selector was in the Both position.

During the second sightseeing flight, while on final approach to ORS, the engine sputtered, lost fuel pressure, and subsequently lost all power. The pilot activated the electric boost pump but observed no change in fuel pressure. Unable to reach the runway, the pilot initiated a forced landing to a field. While maneuvering to avoid power lines, the airplane struck a fence, nosed over, and came to rest inverted. Local law enforcement noted no fuel leaking at the accident site.

Examination Findings

The pilot recovered the wreckage from Orcas Island and moved it to a mainland location without federal oversight, preventing an accurate determination of the fuel quantity at the time of the accident. During recovery, about 10 gallons of fuel were drained. According to the manufacturer, the aircraft’s fuel system retains approximately 3 gallons of unusable fuel.

Examination of the airframe and engine revealed no evidence of preaccident failures or malfunctions. Fuel continuity was established from the main tanks to the carburetor. The engine-driven fuel pump operated normally when tested, and the electric boost pump also functioned without anomalies. Borescope inspection of the main and auxiliary fuel tanks showed no issues.

Pilot Statement

Following the wreckage examination, the pilot communicated that he likely ran a fuel tank dry. He also suggested that air may have been introduced into the fuel lines during maneuvering flight with a low fuel quantity in one tank. When asked about prevention, the pilot stated that topping off both fuel tanks before departure would have prevented the accident.
