What happened
On January 5, 2017, an Neiva EMB-202, registration PT-ULF, operated by Aeroimpar Aviação Agrícola Ltda. - Me, was performing agricultural spraying operations near Fazenda Capilé in Guarapuava, Paraná. After completing three initial spray runs in one area, the pilot moved to a second location to utilize remaining chemicals in the aircraft's hopper.
Upon arriving at the second area, the pilot performed a reconnaissance flight to identify terrain obstacles. However, due to high wind intensity, the pilot decided to execute the spraying passes in the opposite direction of the initial reconnaissance flight. During this maneuver, the aircraft struck an electrical transmission wire. Following the impact, the aircraft traveled approximately 800 meters before striking the ground and overturning.
The investigation
The CENIPA investigation established that while the pilot was properly rated and experienced, the decision to fly in a direction contrary to the reconnaissance pattern disrupted the pilot's mental model of the terrain. The investigation found that the pilot's perception of external obstacles was compromised because the flight parameters deviated from the established plan and the GPS-recorded obstacle points. Additionally, the investigation noted a lack of operational oversight by the operator regarding flight planning and execution.
