What happened
On January 13, 2021, at approximately 20:00 UTC, a Neiva EMB 810-D, registration PT-VIY, was performing a private flight to the Canarana Aerodrome (SWEK) in Mato Grosso, Brazil. The aircraft was carrying a pilot and three passengers. During the landing sequence, the aircraft made its initial contact with the runway, but subsequently climbed and the pilot lost control of the aircraft. The aircraft entered a pre-stall condition and eventually came to a halt beyond the right side of the runway. The impact resulted in substantial damage to the aircraft, though all four occupants escaped without injury.
The investigation
The investigation, conducted by SERIPA VI, was initiated after the occurrence was noted via social media videos. Investigators examined the pilot's credentials and the aircraft's airworthiness status. While the pilot held a valid medical certificate and a private pilot license for single-engine land aircraft, the investigation established that the pilot did not possess the required multi-engine land (MLTE) rating necessary to operate the Neiva EMB 810-D.
Furthermore, the investigation faced several limitations: the pilot's flight logbook was not provided for review, and there were no digital flight hour records available to verify recent experience. Additionally, while the aircraft's airworthiness certificate was valid, investigators could not verify the aircraft's weight and balance status or the maintenance records for the airframe, engine, or propeller due to a lack of accessible documentation.
