What happened
On January 14, 2015, a Cessna A-188B, registration PT-FFH, was engaged in a crop-dusting operation over a corn plantation at Fazenda Cremaq Brasil Agro in Baixa Grande do Ribeiro, Piauí. While performing a low pass to spray pesticide, the aircraft collided with a concrete pole used for supporting power lines. Following the impact, the aircraft traveled approximately 230 meters in an uncontrolled manner before coming to rest in an upside-down position. The collision caused substantial damage to the engine, propeller, wings, and landing gear, and the pilot sustained fatal injuries.
The investigation
CENIPA's investigation established that the aircraft was airworthy, with up-to-date maintenance records and within proper weight and balance limits. The pilot was experienced in agricultural aviation and was familiar with the specific obstacles in the area, having operated in the same location the previous day. A ground assistant had also specifically reminded the pilot of the presence of the poles before takeoff.
Investigators noted that because there were no electric wires currently connected to the poles, the hazard may have been less visually prominent. Furthermore, at the time of the accident, the sun was positioned at the 1 o'clock position relative to the aircraft's flight path, which may have significantly impaired the pilot's visibility of the isolated concrete structure.
